Employee went to climb onto the scalper conveyor and their foot slipped off the conveyor frame and caused them to hit the right side of their ribs on the conveyor frame.

EE was helping another employee bring some hydraulic oil to a piece of equipment after repairing a hyrdraulic line. When exiting a pickup truck, EE slipped on some hydraulic oil on the ground from the busted hydraulic line. He reached out to catch himself on the pickup truck & pulled a muscle resulting in the injury.

Poor work practice, ee removing a maintenance platform by himself, stated he should have gotten a helper. Platform board slipped & pinched his hand (left) on a metal support bar, 2 small skin tears & a contusion to back of left hand, ice packs applied, wound cleaned, checked by medical staff at clinic, returned to work. EE involved post accident evaluation, tox screen negative.

Small exhaust leak - under cab area of Euclid Haul Truck and unusual atmospheric conditions may have allowed fumes to accumulate in cab and exposed operator. Since we have neither seen nor heard of this happening before, this is our best guess.

AFTER CLEANING WORKER WAS WAITING FOR OTHER HELP TO LOWER SCALE TOP BACK TO NORMAL HE REMOVED SAFETY CHAIN AND LI FTED IT TO AVERTICAL POSITION THE METAL SECTION BROKE OVER CENTER AND CAUGHT THIS EE BETWEEN SURFACE AND BASE.
