EE was using a ladder to get a box of grease from a shelf, when EE said the ladder slipped causing EE to fall cutting elbow on the ladder.

EE was moving a roll of flashing material & dropped the roll on a pallet of screen rails, causing a screen rail to whip up & hit EE in the left side of face.

EE was helping another employee bring some hydraulic oil to a piece of equipment after repairing a hyrdraulic line. When exiting a pickup truck, EE slipped on some hydraulic oil on the ground from the busted hydraulic line. He reached out to catch himself on the pickup truck & pulled a muscle resulting in the injury.

Poor work practice, ee removing a maintenance platform by himself, stated he should have gotten a helper. Platform board slipped & pinched his hand (left) on a metal support bar, 2 small skin tears & a contusion to back of left hand, ice packs applied, wound cleaned, checked by medical staff at clinic, returned to work. EE involved post accident evaluation, tox screen negative.
