EMP WAS USING A LONG-HANDLED SHOVEL TO PRY OUT A LOOP OF CABLE OUT OF SOME FROZEN DIRT AND ICE. HE WAS INTENDING TO REMOVE THE CABLE. AS HE PRIED THE BUSHING CRACKED AND HE POPPED HIMSELF IN THE MOUTH WITH THE SHOVEL HANDLE. HIS UPPER LIP SPLIT OPEN LEAVING A LACERATION WHICH TOOK SUTURES

THE EMPLOYEE WAS CRAWLING OUT A DOORWAY TO A POWER CENTER HOUSED ON THE SURFACE OF THE MINE. THE DOOR WAS HINGED AT THE TOP AND WHEN THE EMPLOYEE HAD ENTERED HE WIRED THE DOOR UP. AS HE BACKE D OUT OF THE DOORWAY, HIS HANDS ON THE DOOR FRAME, HE HIT THE DOOR AND IT CAME LOOSE AND STRUCK HIS FINGERS. HE INCURRED 2 FRACTURES ON HIS RIGHT HAND.
