Employee was told to go help other employee work on cone feed belt. Employee got on different belt. Employee was not on the right belt and was not locked out. Employee was terminated for this act as well as drinking on in company transportation. Elko MSHA office investigated this incident on 02/26/2014.

Employee was cleaning rock salt off of grizzly bars when his safety glasses fogged up. He removed his safety glasses. A rock flew up and struck his right eye causing a corneal abrasion. Given a Rx for eye drops and released to full duty 1-18-14.

Employee slipped and fell hurting his Left shoulder. Employee reported it to supervisor. Employee was taken to Doctor treated and released. On 12/03/2013 employee was not getting better so he was taken to doctor again. Employee was taken off work until seeing the doctor on 12/13/2013.
Employee was opening a can of welding rod. Container slipped out of employees hand, cutting right pinky finger at the upper knuckle. Employee was taken to hospital and received 5 stitches. He was then released back to work with no restrictions.
