EMPLOYEE WAS ATTEMPTING TO SECURE CABLE TO END OF DEAD EYE ON 800-D NORTHWEST SHOVEL. AFTER APPLYING TENSION TO CABLE, EMPLOYEE LOST GRIP AND CABLE SNAKED BACK HITTING EMPLOYEE ON LEFT SIDE OF FACE BELOW EYE. INJURY RESULTED IN CUT OT LEFT SIDE OF FACE. TWO STITCHES WERE REQUIRED. EMPLOYEE RETURNED TO WORK THE NEXT DAY. NO LOST TIME.

ON 20 NOVEMBER 1987, EMPLOYEE INFORMED SUPERVISOR THAT ON 11-18-87 HE HAD GOTTEN DIRT/ROCK CHIPS IN RIGHT EYE. RIGHT EYEWAS AT THE TIME SWOLLEN. EMPLOYEE WAS TAKEN TO DOCTOR AND PHYSICIAN CLEA N EYE AND ISSUED MEDICATION FROM HIS OWN INFIRMARY.
