Colowyo Mine Coal

The lab technician was getting ready to collect a coal sample when bolts for an approximately 10-inch guide on the drill sheared, and the guide fell, striking the Lab Tech on top of the right shoulder. Employee was referred to ortho and was prescribed physical therapy.

Warehouse Attendant was attempting to unload two GET Heel Plates from the forks of a forklift. The Attendant slid the plates off the end of the forks and pinched their index and middle fingers on their left hand between the plates and the ground resulting in swelling and bruising, x-ray was negative for fractures.

Security Officer had unused CCTV monitor fall on their head while attempting to retrieve items from post storage space resulting in black eye, stiffness of neck and headache. Security Officer was evaluated by a physician resulting in missing their next scheduled shift and being recommended light duty for several days.

Equipment Operator was assisting to unplug the 007 Crusher. The operator was turning the drive pulley by hand when left ring finger was pinched between the pulley and the belt. An x-ray revealed a fracture in the finger. The employee was given a splint for the finger to be worn while at work. The employee returned to work with no restrictions.

A grass fire was discovered and reported at 13:25 while employees were investigating why the mine site lost power and shortly thereafter, phones. A fire line was immediately cut around the fire and suppression efforts began. It was determined that a large bird had contacted a powerline resulting in the loss of power and the grass fire.

EE was cleaning out cardboard from the back of the blasting truck. EE stepped back towards the rear of the truck and stepped off the bed onto the tailgate. This caused EE to lose balance and fall resulting in EE jamming left wrist when EE tried to catch self.

EE reports that, at the end of EE's shift, EE dismounted a highway haul truck, and EE felt a popping sensation along with a burning sensation and pain in the upper heel area of EE's left foot.

After shoveling spilled coal in the train tunnel for several hours, EE reported feeling pain on the right side of lower back. EE was seen by a physician the following day and given medication for the pain. EE was held out of work until they could be seen for a follow-up with the physician.

A Wagner Equipment Co. field service technician was placing a D11 dozer on jack stands provided by ColoWyo Coal maintenance shop. The tech was positioning stands under the machine using handle. Stand rotated off dolly and pinched finger between handle and belly pan.

EE pinched right thumb and index finger between a gate latch and a door at the train tunnel. X-ray revealed a small fracture to the tip of the thumb. EE was referred to ortho and given a full release to return to work.

A grass fire was reported at approx. 4:49 pm. Mine employees began fighting the fire and requested help from Craig. No buildings, equipment or personnel were in the path of the fire. The fire was extinguished without injury. EMC Sargent of the MoCo sheriff department determined that the fire was due to a bird contacting a powerline.

Employee was moving 55 gallon barrels in the lube room to gain access to the floor drain. While moving the third barrel with the barrel dolly employee felt a sharp pain in lower abdomen. Employee was taken to the local medical facility and treated for an abdominal strain. Employee did not return to work without restriction making incident reportable.

The injured employee was spraying weeds at the mine-site. A gust of wind blew a dust particle into their eye. Irrigation failed to remove the dust. They were taken to the ER and the embedded particle was removed and prescription eye drops were administered. Employee was wearing safety glasses at the time of the injury.

The operator of 249 haul truck backed up to the berm on the dump. The berm sloughed off and the truck became stuck.

The employee was attempting to dump a coal sample. EE felt pain in shoulder. EE reported the incident but felt it was not serious and continued to work. Later, they decided to seek medical attention and was diagnosed with a tear in the shoulder. EE went in for surgery on 4/6/22 and began missing time, making this incident reportable.

A brush fire occurred just outside of the Collom pit. Outside resources were called to assist with controlling the fire. No structures or equipment were threatened by the fire and no one was injured. The cause of the fire is still under investigation.

The employee was attaching a 1 inch oil hose to the fittings on the 116 excavator and felt pain in upper back. There was no accident or trauma. The hose weighs 5 lbs. Employee was not working in an awkward position or overhead.

The employee began to feel ill a few hours after reporting to work. EE was evaluated by an EMT. EE was taken to the hospital for further treatment. Later that day EE was diagnosed as having had a stroke. EE was released to full duty 5/19/21.

The employee reported feeling ill to the lead person, an EMT. The EMT evaluated EE and diagnosed a potential heart attack. 911 was called and the mine's emergency team removed the ill employee from the haul truck EE was operating. EE was later diagnosed at the hospital with a heart attack.

The employee was using a skidder to pull trail cable for a dragline move. EE was dismounting the skidder using 3 points of contact and missed the first step. It was clean, dry and not obstructed. As EE fell, EE struck EE's left side, bruising their ribs. EE was seen for precautionary reasons and was released to work with restrictions.

The injured employee was working on an excavator bucket change in the upper shop yard. Icy spots were identified and a load of rock was spread to mitigate the hazard. Employee was walking to get a hammer when they slid on the ice under the rock and fell to the ground. Employee received a laceration to the right knee that required stitches.

A fire was reported at around 7:45 am. The source of the fire was an old, abandoned underground mine fire. An ember from the old mine must have blown into the dry grass surrounding the hot area on the side of the hill. The fire was mostly contained and extinguished the next day. No one was injured and no mine equipment or structures were damaged by the fire.

The employee was dismounting a lube truck using three points of contact. EE's foot slipped off the step and EE rolled EE's ankle. EE was seen by ER for evaluation. EE returned to full duty for several weeks. EE's ankle began bothering EE again and an MRI was ordered. The MRI revealed a torn ligament and the employee began missing time on 8/4/20, making this incident reportable.

The injured employee was working on a haul truck pm in the pm bay. EE tripped over the oil hose that was pulled out to fill the engine. EE fell to the concrete floor, landing on right side causing injury to wrist. EE was taken to the ER and diagnosed with a fractured wrist.

The injured employee was removing a return idler on the fines belt of the rock crusher. The employees finger was pinched between the idler and the belt frame when it came out of the bracket. The employee was diagnosed with a fractured finger.

A temporary employee was mopping the floor in the lower shop. The employee grabbed the mop bucket handle with right hand and placed left hand under the bucket to dump the dirty water. The employee heard a popping sound and felt pain in left forearm. Employee was released to light duty.

While trying to free a blade that had become stuck, the injured employee got right ring finger pinched between a dozer ripper and a pulling strap. The pinched finger suffered an avulsion of the pad. EE was not released to full duty.

A mechanic was installing the outside right front tire on 287 highway truck. Mechanic was using right arm on the tire dolly and left arm supporting the top of the tire. They felt a sharp pain in left shoulder as they guided the wheel onto the studs. Mechanic was seen by a doctor on 1/7/20 and was released to restricted duty.

The employee was assigned to replace a hydraulic cylinder. EE felt a pop in the left wrist while using two wrenches to loosen a hose from a fitting on the cylinder. EE was seen by a doctor and was released to restricted duty.

A grass fire was reported to the guard shack at 3:36 pm. Mine employees began fighting the fire and requested help from Craig (town). No buildings, equipment or personnel were in the path of the fire. The fire was extinguished without injury.

Employee was descending a short step ladder in the lower shop. EE missed the middle rung on the ladder and caught self with EE's right hand. EE felt fine at the end of shift but felt soreness in EE's right shoulder the next morning. On 7/17 the employee was diagnosed with a torn rotator cuff and began missing time on 7/19.

A brush fire started on the evening of 9/9/18. The fire was most likely caused by lightning as there are no mining activities near the point of ignition. This brush fire is being reported as it took longer than 30 minutes to extinguish the fire from the time it was reported. No one was injured while extinguishing the fire.

Northwest Tool Employee injured while attempting to connect an airline to a valve. The Valve failed causing fragments to stick into employee's left arm. Medical treatment required to remove small particles and released same day to full duty.

A mechanic injured a knee dismounting a pickup truck. Employee sought medical attention on 1/19/18 and was not released to return to work.

The injured employee placed a hand on the outrigger of a service truck. Another employee retracted the outrigger, pinching a finger. Injured employee received 5 stitches.

Lightning started a brush fire at Collom. The fire burned for over a half hour so it was reported to MSHA. The fire was discovered at 2:50 pm and was extinguished by a rain storm at 3:39 pm. The fire was in a remote location and no structures, equipment or personnel were threatened by the fire.

EE working on Komatsu WA1200 Front Loader. EE was installing an intake coupling (or intake boot) between an air box and first piece of intake pipe. EE experienced pain in right thumb. Right thumb has torn ligaments. Two prescriptions, a splint, and work restrictions were given at the clinic. Surgery is required to reconnect ligaments.

A miner who had undergone treatment for cancer believes they broke a rib while stretching. The miner was seated in the breakroom before beginning work and raised their arms above their head, stretching and felt a tingling in their back/side. Later was diagnosed with a fractured rib. It is unclear if this was the event that caused the fracture.

An employee fueled 609 backhoe. While mounting the machine employee felt a pop in right shoulder. Employee was evaluated by a Dr. on 4/2/17 and was not released to return to work, making this incident a lost time.

A temporary employee slipped while pulling wires from underneath the MCC room on a dragline. When ee attempted to catch self, ee injured right shoulder. The employee was not released to return to work and began losing time on 12/31/16.

A brush fire was reported near the red dog corner around 4:05 pm. I reported the fire to MSHA at 4:27 pm. Fire was out at 6:00 pm. No injuries or equipment damage occurred due to this fire. Vegetation conditions in the county were very dry.

Alsco Sales rep was lifting rugs in vehicle. Injured coughed and felt a pop and burning pain in side.

The injured employee tripped over a chunk of mud and fell on right side. EE initially refused treatment. A follow up exam was conducted and EE was not released to work for next scheduled shift (5/20/16).

The injured employee was pulling a hose off of a water truck. The hose coupling hit him in the mouth as he pulled it off of the truck. The coupling split his lip, which required stitches.

The employee reported to his supervisor that he was feeling ill. He was transported off-site for evaluation. Later that day he was diagnosed as having heart attack.

While dismounting a piece of equipment an employee twisted his ankle. He was released to restricted duty.

A mechanic grabbed a structural support beam while climbing into the center section of the machine. In doing so, he dislocated his right shoulder. He was not released to return to work making this a lost time incident.

A contractor employee tripped while engaged in rail road track repair. The employee was working at ground level when he fell. He was diagnosed with a hip fracture.

While dismounting a dozer an employee slipped and hit his safety glasses on a handrail. The safety glasses caused two lacerations on his face which required stitches, making this incident reportable.

While installing a ripper tooth an employee struck the keeper pin with a 2 lb hammer. The strike was off center and the hammer glanced off the pin and hit his left thumb. The result was a laceration which required six stitches, making this incident reportable.

While loading a haul truck, a piece of coal fell off the bucket of a front end loader. The coal bounced off of the hand rail and broke the haul truck operator's side window. The operator received a contusion to his left shoulder. The ER physician did not allow him to return to work making this incident reportable.

The employee was attempting to stand from a seated position and dislocated his left shoulder. There was not trauma incident involved, the employee states that he has a history of shoulder problems.

Employee was dismounting a utility truck, missed the last step and fell to the ground. He did not return to work, making this accident reportable.

The employee was pulling on a wrench to loosen a bolt and felt a pop in his right shoulder. He received a light duty release making this incident reportable.

While attempting to repair to a braided hose mechanics left ungloved hand slipped causing left pinky finger to catch on exposed wire braid lacerating left pinky finger. 4 stitches administered as a result.

Employee was taking tension off of the 3 3/4 reeving cable where a shackle had become wedged between the sheave and housing. Employee was using a 1/2" chain and 3 ton come-a-long to pull the tension out of the reeving cable when the chain slipped and struck him on the right hand.

Employee kicking in drill holes reported twisting knee. Off site doctor conducted exam, no injury but follow up testing scheduled. Follow up results of exam on July 3, 2012 showed a tear in employees right knee. Unknown at time of report if any further action to be taken.

The employee was moving a rolling ladder platform on the shop floor when the employee felt a pain in right arm/shoulder. Employee finished shift and worked next shift before reporting injury the morning of 4/26/2012. Initial medical exam restricted work duties until follow-up doctor exam is scheduled. Determination of work status unavailable at time of report.

The injured employee was cleaning coal basket, turned to leave area and hooked his right arm on a rail, which "wrenched " his arm backward. The company sent the employee to an emergency room and the Doc. told him it was likely a rotator cuff tear. The employee took the next shift off to see a specialist. The employee stated that this was an aggravation of a pre-existing condition.

While grinding a wheel motor commutator, the grinding stone wedged in the commutator causing the grinding stone to spin around pinching the left index finger between the grinding stone and the commutator. The pinching caused a laceration requiring 5 sutures.

While the employee was dismounting a haul truck he lost his footing and slipped off the truck running his hand along the handrail and getting his right lower palm caught on a bolt resulting in a lacertation to the lower section of the right palm.

While employee was lining up a rock knocker on a lube truck for installation his right middle finger was pinched when the rock knocker and pin (bushing) slipped.

Employee was closing the door on the BE 1300W dragline that enters the PCL room when his glove caught on a metal burr on the door latch which caused the employees left index finger to be smashed in the door. This resulted in pinching off of the flesh of the tip of the finger.

While utilizing 2-1500 lb magnets to pick and place a dozer corner cutting edge, a mechanic recieved an angular laceration to jaw. A pry bar used to position the cutting edge holes with the holes of the dozer blade swung up when 1 magnet failed. The mechanic went to the emergency room and recieved 3 sutures and recieved a full release.

On 10/28/08 the employee stepped on a piece of coal, lost balance and fell on their right arm and back. The employee followed up with a doctor for futher examination. A cyst was found in the right shoulder (pre-existing). The employee chose not to do any medial treatment at that time. On 3/30/09 the employee notified that he would like to have surgery on the right shoulder.

Employee was removing a cullet from a pin. Employee lifted the pin off the ground using a remote control and shop crane (wearing no gloves). Two other workers were using a 8# metal hammer to "tap" cullet free from the pin. Injured employee moved 8' away from work area to lay the remote down. A metal sliver came and imbeded in employees top side right hand.

EE was using the hydraulic press to flatten a push plate. He had set the plate and was pressing when spacer dislodged and hit him in the face. He has large cut across right cheek bone and upper jaw is dislodged from skull.

Contractor truck driver became ill while sitting in the cab of his truck doing paperwork. Paramedics responded; possible heart attack. Driver transported to hospital in Craig, CO for evaluation.

Field Service Mechanic was attempting to access the left side cab door of a CAT 16G motorgrader to repair a faulty door latch. The ee slipped from the ladder and landed on the ground (+/- 30"). This action resulted in a fracture of the right tibia just below the knee.

Employee was involved with the changing out new drag ropes. The employee chose to step out onto the drum without fall protection. While reeving the new rope onto the drag reel, the employee fell off the drum. The employee fell ~11 ? feet. Injuries included 7 fractured ribs and a fractured scapula.

The employee was checking cone crusher shear pins when he felt a "pop" in his left knee. We did not know this would be a reportable incident until 3-23-06 when he missed his next scheduled shift.

The employee was driving a pick up on the train loadout access road. As he rounded a corner, his pick up was in the wrong lane for reasons that are not yet understood. The pick up collided with a 50 ton tractor trailer coal hauling rig. The employee in the pick up sustained a fractured left femur and right knee, as well as lacerations, contusions, and soft tissue injuries.

A scaffold collapsed while being dismantled. A welder on the lowest level fell about four feet to the ground, sustaining a lower leg fracture and damage to the ligaments in the right ankle.

While working on a LeTourneau front end loader, the employee sustained a laceration to his right middle finger that required stitches to close. The injury occurred when the employee's hand was struck by a falling hydraulic pump. The employee was released to return to full duties.

EE states he was crawling out of a Bobcat loader. He felt himself falling. While attempting to catch his balance his right hand hit the corner of the door frame. The nail on his right hand was partially torn off causing a laceration that required 4 stitches. X-ray determined a hairline fracture of the tip of the right hand thumb.

EE states that while placing a radiator into dozer, his hand was pinched between the radiator and the fan pulley on the front of the engine. EE sustained a laceration to the small finger on his small finger on his left hand that required four sutures to close.

on 1-5-05 ee stated he slipped and fell on some snow and ice while cleaning fines around the rock crusher area and felt pain in his left shoulder. ee was taken to the dr and was released to return to full duty. on 2-15-05, ee was diagnosed and underwent surgery for a torn rotator cuff of his left shoulder.

EE STATES THAT WHILE GETTING OUT OF HIS TRUCK HE FELL TO THE GROUND. EE SUSTAINED A BRUISE TO HIS LEFT HIP, A BUMP TO THE TOP OF HIS HEAD (CONCUSSION) AND A SORE SHOULDER.

EE CLAIMS THAT WHILE INSTALLING A DRIVELINE ON A 793 HAUL TRUCK, THE WRENCH HE WAS USING FAILED CAUSING PAIN IN HIS LOWER BACK.

EE STATES HE WAS ATTEMPTING TO SEPARATE TRAFFIC CONES THAT HAD FROZEN TOGETHER WHEN HE FELT A POP IN HIS RIGHT SHOULDER WHICH WAS ACCOMPANIED BY PAIN. EE RETURNED TO FULL WORK DUTIES ON JAN 2, 2004 AFTER REGULARLY SCHEDULED DAYS OFF. DR DIAGNOSED SOFT TISSUE INJURY TO RIGHT SHOULDER AND BICEP MUSCLE.

WHILE WORKING WITH A BUCKET TRUCK HIS HAND WAS PINCHED BETWEEN THE BUCKET AND A CONDUIT HE WAS WORKING ON. HE WAS TAKEN TO A DOCTOR AND SUTURES WERE APPLIED. NO LOST TIME.

WHILE MOUNTING DOZER HE SLIPPED & FELL TO TRACK LEVEL HURTING LEFT SHOULDER. INITIAL CHECK BY DOCTOR SHOWED NO INJURY. EMPLOYEE WAS RETURNED TO DOCTOR ON 12/3/03 & A HAIRLINE FRACTURE WAS FOUN D ON TIP OF LEFT SHOULDER.

EMPLOYEE STATES THAT A PARTICLE OF DIRT BLEW INTO HIS EYE WHILE WORKING AT 102 DRAGLINE. EMPLOYEE WAS TAKEN TO THE EYE DOCTOR WHERE A SMALL PARTICLE OF DUST WAS REMOVED AND SOME ANTIBIOTIC WAS APPLIED. EMPLOYEE RETURNED TO WORK SAME SHIFT.

EE STATES THAT WHILE MOVING A JACK HAMMER HIS RIGHT INDEX FINGER BECAME PINCHED BETWEEN THE JACK HAMMER AND SECTION OF HANDRAIL. EE SUSTAINED A LACERATION TO THE TOP OF HIS FINGER AND A SEVERE D TENDON THAT REQUIRED SURGICAL REPAIR AND STITCHES.

HE STATED WHILE WELDING HE NOTICED HIS EYE WAS IRRITATED, EMT FLUSHED IT FOUND NOTHING. AT THE END OF SHIFT IT WAS BOTHERING HIM, HE WENT TO DOCTOR, DR REMOVED SMALL PARTICLE AND GAVE HIM SOME ANTIBIOTIC OINTMENT.

EMPLOYEE STATES THAT WHILE OPERATING A ROAD GRADER, THE BALL JOINT ON THE FRONT OF THE GRADER BROKE CAUSING A SUDDEN JOLT THAT RESULTED IN PAIN IN HER LEFT SHOULDER. EMPLOYEE WAS TAKEN TO A DOCTOR THAT HAS PRESCRIBED RESTRICTED WORK.

EE STATES THAT WHILE OPERATING A RUBBER TIRE DOZER HE HAD STOPPED, LEFT THE CAB AREA AND WHILE OUTSIDE, A GUST OF WIND BLEW A PARTICLE OF DUST INTO HIS EYE. AFTER A COUPLE OF DAYS HIS EYE BECA ME SWOLLEN AND HE WENT TO THE DR. WHERE HE WAS DIAGNOSED WITH A BACTERIAL INFECTION. EYE DROPS WERE GIVEN. ( EE WAS WEARING SAFETY GLASSES).

EMPLOYEE STATES THAT AS HE WAS GETTING UP FROM A MECHANIC'S "CREEPER", HE SLIPPED CAUSING HIS RIGHT KNEE TO STRIKE THE CONCRETE FLOOR. THAT EVENING HIS KNEE SWELLED DUE TO ACCUMULATION OF FLUI D. ON 6/28/02 A PHYSICIAN DRAINED FLUID FROM THE EMPLOYEE'S KNEE.

EE STATES THAT WHILE OPERATING A POWDER TRUCK FOR THE BLASTING CREW, A GUST OF WIND BLEW A PARTICLE OF DUST INTO HIS RIGHT EYE. EE WAS TAKEN TO A DR WHERE THE PARTICLE WAS WASHED OUT AND AN OU NTIBIOTIS OINTMENT WAS APPLIED EE WAS WEARING SAFETY GLASSES.

EE STATES THAT WHILE WALKING ACROSS UN-EVEN GROUND, HE SLIPPED AND FELL CAUSING WEIGHT TO SHIFT TO HIS RIGHT LEG. EE FELT PAIN IN HIS RT. KNEE THAT WAS DIAGNOSED AS A TORN QUADRICEPS MUSCLE. SURGERY WILL BE REQUIRED TO REPAIR HIS KNEE.

EE STATES THAT HE STRUCK HIS RIGHT SMALL FINGER AGAINST FENDER OF A SERVICE TRUCK WHILE REMOVING A CHOCK BLOCK. THE IMPACT CAUSED TRAUMA TO SOFT TISSUES @ DISTAL END OF FINGER. FIRST AID WAS U SED TO TREAT INJURY INITIALLY, BUT THE CONDITION DID NOT IMPROVE. MEDICAL TREATMENT OCCURRED ON 5/21/02 WHEN A PHYSICIAN INSERTED A METALLIC PIN IN THE FINGER TO TREAT THE INJURY.

EE STATES THAT WHILE CLIMBING ABOARD HIS HIWAY TRUCK, HIS FOOT SLIPPED CAUSING HIM TO FALL DURING THE FALL HIS LEFT HAND HIT THE DOOR JAM CAUSING A FRACTURE TO THE FIFTH METACARPAL BONE.

EE STATES THAT WHILE USING AN AIR ACTUATED TORQUE WRENCH, HIS FINGER WAS PINCHED BETWEEN THE WRENCH AND A PIECE OF STEEL. EE WAS TIGHTENING THE LUG BOLTS ON A 190 TON TRUCK. THE INJURY RESULTE D IN A SMALL PIECE OF BONE BEING REMOVED FROM THE TIP OF THE RING FINGER OF HIS RIGHT HAND.

EE STATES THAT WHILE MOVING SOME TRAIL CABLE BY HAND, SHE SLIPPED AND FELL CATCHING HERSELF WITH HER RIGHT HAND. EE WAS TAKEN TO A DR. WHERE IT WAS DETERMINE SHE HAD BRUISED OR SLIGHTLY CRACKE D A BONE IN HER WRIST NEAR HER THUMB.

EE STATES THAT WHILE WASHING 671 DOZER HE LOST HIS FOOTING AND FELL FROM A STANDING POSITION TO A SITTING POSITION LANDING ON THE TRACK. EE COMPLAINED OF PAIN IN HIS BACK AND NECK. THE DR. PLA CED HIM ON RESTRICTED DUTY.

EMPLOYEE'S RIGHT HAND WAS CAUGHT BETWEEN A PUMP AND GEARBOX ON 538 OVERBURDEN DRILL. HE CONTINUED TO WORK UP UNTIL 8/17/01 WHEN DUE TO SWELLING IN HIS RIGHT HAND HE WAS TAKEN TO THE DOCTOR. IT WAS DETERMINED THAT HE HAD SUSTAINED A FRACTURE TO THE #4 METACARPAL BONE OF THE RIGHT HAND. THEFRACTURE IS STABLE; NO TREATMENT NECESSARY.

WHILE REMOVING A SEAT FROM THE 151 TEREX LOADER HIS FINGER WAS PINCHED BETWEEN THE SEAT & A HANDRAIL CAUSING A LACERATION TO THE TIP OF HIS LEFTINDEX FINGER WHICH REQUIRED SUTURES TO CLOSE.

TIGHTENING BOLTSON GEARS CASE WITH A HAMMER WRENCH A 20 LB HAMMER USED TO HIT THE HAMMER WRENCH. THE HAMMER GLANCED OFF THE WRENCH AND HIT THE MAN ON THE FINGER.

EE STATES THAT WHILE HELPING ANOTHER EE REPAIR A DRAGLINE LINK ON A DRAGLINE BUCKET, THE OTHER EE SWUNG A SLEDGE HAMMER WHICH THEN SLIPPED AND SRUCK HIM IN THE MOUTH. THE RESULTING INJURY WAS A BROKEN TOOTH AND THREE STITCHES TO HIS LIP.
