EMPLOYEE PULLED ON LARGE PULLEY ON VIBRATING GRIZZLEY. PULLEY IS ON SHAFT WITH COUNTERWEIGHTS. HIS LEFT HAND WAS ON THE BELTS & AS HE RELEASED THE PULLEY, THE COUNTERWEIGHTS STOPPED THE PULLE Y IN IT'S ORIGINAL POSITION. HIS LEFT HAND WAS STILL ON THE V BELT. THE BELT PULLED HIS FINGERSINTO THE PULLEY ON ELECTRIC MOTOR. THE EDGE OF HIS MIDDLE FINGER & RING FINGER WERE SEVERED.
