Audiogram on 2/25/26 showed a reportable hearing loss and WDS does not know if they receive a letter and employee does not recall receiving one. The company WDS hired was supposed to mail reports to each person.

EE had picked up on the leg of the conveyor with both hands, and removed the pin. When putting it back down, the ledge of the conveyor slipped and pinched tip of EE's middle finger on the left hand. This cut EE's finger open and caused it to bleed.
EE was taking large rocks out of the cone crusher and setting them on the ledge so they can throw them out and one of the rocks rolled off the ledge and came in contact with EE's 3rd/Ring Finger on the right hand.
