Cone crusher got plugged up with rocks, employee was getting rocks out of cone crusher, footing slipped and fell over ring of cone, ended up going to Urgent Care.
Closing lid glove got caught pull hand down between frame and lid. Middle finger fracture and laceration. Left hand

Employee was working on a piece of equipment when a breaker was tripped by another employee flipped the breaker while the employee's hand was in the area of work(belt)and it cut the pad of the employee's right middle finger tip. Employees were not following Lock Out/Tag Out procedures.
