HE WAS REPLACING BELT ON CONVEYOR BELT AND CLIMBED OVER THE TAIL PULLEY AND FELL ON 2" ANGLE IRON ON FRAME. HE LANDED ON HIS LEFT SIDE OF BODY BRUISING LEFT UPPER RIB AREA. HE DID NOT SEE DR. DAY OF INJURY AND WENT IN FOR MEDICAL ATTENTION ON 6-8-93.
