Employee was injured when he slipped and fell when he was climbing into his drilling equipment. Snow and ice conditions on the steps caused the employee to slip. He fell and hit his ribs and head when he fell. Employee was taken to a medical facility and was diagnosed with a fractured rib and bruising under the eye.

Finished working on 988F, the loader pulled away employee continued picking up tools. As he reached down to get the tools the wind blew dust and fiber in to both eyes. Employee was taken to Union County General Hospital in Clayton New Mexico, 301 Harding Street, Clayton New Mexico.

EE cleaning mud from under conveyor slipped & fell back & hit his head. Cut to back of head. Bar EE was using hit him in chest. Foreman called Moriarty ambulance & EE was transferred to University Hospital (Albuq) to be checked out. It's Southway policy that any EE hurt be checked out by a Dr or ER. EE was wearing fall protection, taken off at hospital.
