BEARING WENT OUT ON CRUSHING PLANT.CRUSHER WAS SHUT DOWN.WHEN REPAIRING ANOTHER EMPLOYEE WAS AHMMERING WITH A HAMMER ON THE SHAFT TO DRAW IT DOWN.A PICE OF THE SHAFT CHIPPED OFF AND FLEW INTO THE INJURED EMPLOYEES EYE.
