EMPLOYEE HOOKED A CHAIN ON A GUARD AND DID NOT PUT A BOLT THROUGH TO SECURE THE CHAIN TO THE GUARD. WHEN HE LIFTED THE GUARD THE CHAIN SLIPPED OFF AND THE GUARD FELL AGAINST HIS LEG BRUISING IT.
