Vigo - Cypress Mine LLC · Struck against stationary object

Employee stated he was repositioning the gear box for the radial stacker which was located on the shop welding table. His left hand slipped and struck a small burr on the gear box resulting in a laceration requiring 12 sutures to the ring finger of the left hand.

Employee stated he was in the process of spreading a tarp over the 1800 Hitachi Shovel car-body to keep the rain off the area where the disassembly with was being performed. As he took a step to spread the tarp he slipped on the slick surface causing him to fall backwards striking the middle of his back area on the shovel's roller.

Employee was using a torch to cut bolts from #6014 dozer track pads when a particle of dirt got under his eye protection and entered his right eye. The employee was taken to Epworth Conenient Care where he was given a prescription eye wash and released to full duty to return to work.

EE was driving #6005 85 ton Komatsu truck into the south ramp of #2 pit. He stated the throttle stuck which caused him to drive into rocks at the digging face of the 1800 Hitachi shovel. The impact caused him to strike the visor in the cab which caused a scalp laceration. He also had a contusion to the left hand. He was wearing his seat belt which prevented a more serious injury.

EE stated he was cleaning front windshield on loader, when he slipped and fell on his left side(same level), causing a sprain to the left shoulder. EE said that mud accumulated on his boots, which caused the accident.

EE backed 85 ton haul truck into bucket of 1800 Hitachi Shovel causing a rock to fall into the truck bed. EE alleges the impact of the rock caused a whiplash motion to his neck. There is no lost time from the accident, however he is on restricted duty.

EE STATED THAT WHILE DISMOUNTING THE MAINTENANCE LUBE TRUCK, HIS FOOT SLIPPED OFF THE BOTTOM STEP OF THE OPERATORS CAB CAUSING HIM TO FALL TO THE GROUND. HE STRUCK HIS LEFT ELBOW ON THE GROUND RESULTING IN A CONTUSION. BY DR.'S ORDERS HE WAS RESTRICTED FOR ONE DAY.

EE HAD BEEN LOADED WITH MUD BY THE 1800 HITACHI SHOVEL. HE WAS STARTING OUT THE RAMP WHEN THE MUD TRANSFERRED TO THE BACK OF THE BED. THIS CAUSED THE 777 TO RAISE IN THE FRONT. WHEN THE TRUCK CAME DOWN, THE SUDDEN STOP CAUSED HIS HEAD TO WHIP FORWARD & STRIKE THE STEERING WHEEL (10 SUTURES REQUIRED).
