EE was changing a turbo on a haul truck. EE was using a scraper to remove the gaskets from the turbo and needed to change the blade. After changing the razor blade on the scraper, the employee was trying to tighten the set screw with an O-ring pick. EE's hand slipped and caused the razor blade to cut EE's middle finger on EE's right hand just above the knuckle on EE's right hand.

Employee was changing bucket teeth on a PC 300 excavator bucket on October 19th 2016. The employee states EE felt a pain in EE's right shoulder while swinging sledge hammer and needed to stop for a minute. EE requested medical treatment on Nov. 4th. EE was diagnosed with sprained right shoulder.
