HE WAS HOLDING THE FLUSHING HEAD WITH B OTH HANDS. HE MISJUDGED SWING IT & CAUGHT HIS FINGER UNDER IT. THE FLUSHING IS STEEL & WEIGHTSABOUT 20LBS. RIGHT 5TH FINGER LACERATION/FRACTURE 1ST JOINT. DISLOCATION 2ND JOINT

EMPLOYEE WAS WALKING ON PARTIALLY DRIED NEAT CEMENT. HIS RIGHT FOOT SLIPPED, BUT HIS LEFT FOOT DID NOT. AS HE FELL, HIS LEFT ANKLE TWISTED INWARD RESULTING IN A FRACTURED FIBULA.
