EE HAD SWING THE DRILL HEAD OUT TO INSTALL THE RIB BOLT IN THE #1 ENTRY. WHEN HIS DRILL STEEL CONTACTED THE TOP IT DISLOGED A PIECE OF DRAW ROCK THAT FELL STRIKING HIM ON THE LEFT ARM, JUST AB OVE THE ELBOW. THE EE SAW A DR ON 4-15-02 WHO ATTHAT TIME PLACED HIM ON RESTRICTION.
THE INJURED EE HAD GONE TO #4 ENTRY TO OBSERVE THE CONTINUOUS MINER OPERATOR TAKE THE FIRST CUTOUT OF THE PILLAR BLOCK ON THE LT. SIDE OF THE ENTRY. AFTER ARRIVING AT THE MINER, A PIECE OF ROCK 8 1/2'LONG X 27"WIDE X 10"THICK ROLLED OUT OF THE RIB CATCHING THE EE ON THE RT. CALF CAUSING A FRACTURE OF HIS RT. ANKLE.

THE FIREBOSS FOUND THE FALL AT 1205 AT SPAD NO. 435, #1 ENTRY APPROX 1800' OUTBY THE FACE AREA. THE FALL MEASURED APPROX 80' LONG X 18' WID4E AND 6' HIGH. THERE WAS NO PROPERTY DAMAGE AND VENT ILATION WAS NOT AFFECTED.

HE PICKED UP A 5 GAL CANOF OIL TO MOVE IT TO HIS OTHER SIDE AND HIS KNEE SLIPPED IN THE MUD CAUSING HIM T O TWIST HIS LEFT KNEE. THE EE WORK WITH NO RESTRICTIONS UNTIL 6-27-01 AT WHICH TIME A SPECIALIST PLACED HIM ON RESTRICTED DUTY.
THE FIREBOSS AND MSHA INSPECTION FOUND THE FALL WHILE MAKING AIRWAYS AT APPROX 645 PM. THE FALL MEASURED 19' WIDE BY 70' LONG BY 4-6' HIGH. LOCATION #6 ENTRY SS# 256 OUT BY FALL 4500'.

HE HAD DRILLED THE FIRST HOLE OF THE SECOND ROW AND WAS RE MOVING THE STEEL WHEN A PIECE OF DRAW ROCK THAT WAS UNDETECTED FELL STRIKING HIM ON THE LEFT THUMB. THE ROCK WAS APPROX 23" X 18" X 8" THICK.

WHEN EE STARTED DRILLING THE HOLE THE BIT BROKE LOOSE A PIECE OF ROCK APPROX 2' X 1.5' X 2.5" THICK STRIKING IM ON THE LEFT LEG JUST ABOVE THE KNEE.

HE WORKED WITH NO RESTRICTIONS UNTIL 8/11/00 AT WHICH TIME SURGERY WAS PERFORMED ON HIS LEFT KNEE.
