Employee was helping string a conveyor belt. When pulling slack out of the roll with a rope, the knot secruring it came loose, causing them to fall onto a steel beam. Employee has a small fracture to the ninth rib.

Employee was walking around a skid steer and tripped over the corner of the bucket, causing him to fall to the ground. Employee requested to be seen by physician for leg pain. Employee was seen by a doctor for pain to his upper left leg. Physician determined there was no broken bone and restricted employee to limited walking. Employee was released to full duty on 5/4.

HE HAD JUST FINISHED LOADING THE SANDBLASTER WITH SAND WHEN THE SANDBLASTER, WHICH WAS ON A SLIGHT INCLINE, BECAME TOP-HEA5Y AND STARTED TO TOPPLE OVER. THE EMPLOYEE WAS IN THE LINE OF THE FALLING SANDBLASTER AND MADE AN EFFORT TO PUSH THE MACHINE IN ANOTHER DIRECTION, BUT IT FELL ON HIM. THE EMPLO
