EE was removing the impellor off of a water pump. The impellor fell off of the shaft and onto EE's right arm. It cut EE's forearm open leaving an approximately 3" long wound.

Employee was conducting a pre-inspection of a dozer and was climbing off the machine when employee slipped and fell on the left hip.

Employee was cutting bolts from underneath a screen, raised up knocking hard hat off and hit head on the corner of a piece of metal.
Employee was cutting a trelix pad with a utility knife, knife slipped and sliced through glove on left hand, cutting a slice in the palm of EE's hand.

Employee was moving and throwing rocks by hand and twisted wrong, straining back.
Trimming a belt from a stacker. Knife came through the belt when finishing the cut and cut right hand through the glove employee was wearing.
Attempting to straighten a roll of belting and apparently strained back. Employee did not notify the employer until 06/12/2015 stating that it did not hurt until several days later. and therefore did not seek medical attention until the evening of 06/12/2015.

Employee was helping to fabricate a conveyor in the shop. A clamp, which was holding up a piece of metal failed. The metal fell on the employee's right foot, causing a fracture of the right great toe. There was no time lost.
