EE WAS INSTALLING A HYDRAULIC HOSE ON A 560 LOADER. EE PULLED ON THE HOSE AND FELT PAIN IN HIS WRIST; HAD SLIGHT SWELLING IMMEDIATELY. EE DID NOT INFORM ANYONE UNITL FRIDAY, 4/1/05 & EE REFUSED TO SEE DOCTOR. ON MONDAY, 4/4/05, EE STILL COMPLAINED OF PAIN, BUT STILL REFUSED TO SEE DOCTOR. ON TUESDAY, 4/5/05, EE ASKED TO SEE DOCTOR. TAKEN TO DOCTOR 4/6/05.

EMPLOYEE STATES HE RAN OUT OF ACETYINE ON WELDING TRUCK WHILE WORKING ON PROJECT. TOOK OUT EMPTY TANK AND WENT TO LIFT IN A FULL ONE, ALONE. WHILE LIFTING, FELT PAIN IN LOWER BACK. DID NOT REPORT INJURY TO SAFETY COORDINATOR UNTIL AFTER NOON ON 10/22. CAME IN TO WORK ON 10/23. LEFT EARLY BECAUSE OF BACK PAIN. DID NOT COME IN 10/24. DR DIAGNOSED LOWER BACK MUSCLE STRAIN.

WHILE EXITING HAUL TRUCK EMPLOYEE MISSED STEP WITH L FOOT, TURNED LOOSE OF HAND RAIL & FELL TO GROUND, LANDING ON HIS L SIDE. HE WAS ATTEMPTING TO CLIMB DOWN A LADDER IN FORWARD POSITION, INST EAD OF BACKWARDS. ACCIDENT RESULTED IN; BROKEN PELVIS, FRACTURED SHOULDER & E BROKEN RIBS. HAUL TRUCK WAS PARKED.

HAD DRIFTER ALL THE WAY TO TOP OF DRILL CHANGING DRILL STEEL. CHAIN THAT DRIFTER RIDES ON BROKE. DRIFTER FELL, HITTING DRILL STEEL. DRILL STEEL FELL OVER KNOCKING HIM TO THE GROUND. IN THE FA LL HE FRACTURED HIS LEFT HIP.
