EMPLOYEE WAS USING A 6" DIESEL WATER PUMP TO FILL A 10,000 GAL WATER TRUCK. THE ACCIDENT OCCURED ON HIS 3RD FILLUP. THE FOOT VALVE ON THEINTAKE LINE BECAME CLOGGED WITH DEBRIS. THE PUMP QUIT WORKING AND THE WATER INLET PUMP CHAMBER HEATED UP. EMPLOYEE THOUGHT THE PUMP NEEDED TO BE PRIMED TO WORK AGAIN. HE REMOVED THE CHAMBER CAP, WHEN HE DID HOT WATER SPRAYED OUT AND BURNE

EMPLOYEE WAS REFUELING A HIGHWALL DRILL. THE FUEL HOSE CAME LOOSE FROM THE FUEL NOZZLE. HE JUMPED FROM THE DRILL DECK TO THE GROUND TO STOP THE FLOW OF FUEL. HE COMPLAINED OF PAIN IN HIS RIGHT KNEE AFTER HE JUMPED. 3 1/2-4 FT.

EMPLOYEE ALLEGES HE STRAINED HIS KNEE ABOUT A MONTH AGO BUT DID NOT REPORT IT TO ANYONE. HE STATED HE WAS CLIMBING DOWN THE LADDER OF A 777 CAT TRUCK WHEN HE HURT HIS RIGHT KNEE. HE WENT TO TH E EMERGENCY ROOM ON 12/27/94 AND WAS X-RAYED AND GIVEN A BRACE.
