Speedway Sand & Gravel · Over-exertion (Not Elsewhere Classified)

Hurt ankle on 2/12/25. Did not report injury till after EE went to ortho on 2/17/25. Claimed to step off haul truck and foot slipped and twisted ankle a bit 5 days later. Ortho found that there was a fracture in the foot. After our investigation EE was put on workmen comp for 24 days until EE was released back to full duty.

W & W Gravel Co., Inc. · Fall onto or against objects

EE WAS ATTEMPTING TO MANUALLY PLACE A FENCE POST THAT HAD ALREADY BEEN DUG WITH AN EXCAVATOR, WHEN A FLANGE ON THE FENCE POST CAUGHT HIM ON THE FACK SIDE OF HIS RIB CAGE. THIS PULLED HIM DOWN INTO THE POST HOLE.
