BACK AREAS OF THE MINE WERE BEING CLEANED UP AND REBOLTED PRIOR TO PRODUCTION BEGINNING. A PORTION OF AN OLD ROOF FALL WAS BEING CLEANED UP AND REBO/LTED. THE EMPLOYEE WAS BOLTING ROOF IN AN AREA THAT WAS APPRAOX. 16' HIGH AND AS HE TIGHTENED A ROOF BOLT HE HAD JUST INSTALLED, A SLUG OF ROCK FELL FROM AROUND THE BOLT AND STRUCK HIM ON THE SHOULDER.
