A ROOF FALL OCCURRED SOMETIME OVER THE WEEKEND AND WAS FOUND ON THE PRESHIFT EXAMINATION BY THE EXAMINER.THE FALL OCCURRED AT SURVEY STATION #69,3 BREAKS OUTBY THE FACE OF THE NO 4 HEADING.THE FALL MEASURED APPROXIMATELY 20'WX20'LX4'-6'THICK AND FELL ABOVE THE ROOF BOLT ANCHORAGE ZONE.SECTION WAS NEARING OUTCROP WHEN FALL OCCURRED.

EMPLOYEE WAS DRILLING A 5 FT TEST HOLE AND STARTED TO ADD ANOTHER PIECE OF STEEL, WHEN THE PIECE OF PUSHER STEEL UNCOUPLED FROM THE FINISHER PIECE THE FINISHER PIECE OF STEEL FELL AND CAUGHT HIS LEFT THUMB AGAINST THE HEAD OF THE BOLTER. THE THUMB NAIL HAD TO BE REMOVED.
