THE EMPLOYEE WAS GOING INTO THE SCALE OFFICE. HE TRIPPED ON THE BOTTOM STEP OF THE STAIRS AND SPRAINED HIS RIGHT AKNLE.
THE EMPLOYEE WAS LOADING CRUSHER WEAR PLATES IN TO A LOADER BUCKET. AS HE PICKED ONE UP AND TURNED TO PUT IT IN THE LOADER BUCKET, HE HAD A SHARP PAIN IN HIS BACK.

LOADER WAS BEING USED FOR A MAN LIFT, PARKED ON A HILL BY A VERTICAL WATER TANK,WHERE THEY WERE INSTALLING A SIGNT GLASS ON THE TANK. OPERATOR STATED THAT THE LOADER HAD A MECHANICAL FAILURE L ETTING ALL AIR OUT OF SYSTEM CAUSING THE BRAKES TO FAIL. WHEELS WERE NOT CHOCKED.
LOADER WAS BEING USED FOR MAN LIFT, PARKED ON AHILL BY A VERTICAL WATER TANK, WHERE THEY WERE INSTALLING A SIGHT GLASS ON THE TANK. OEPRATOR STATED THAT THE LOADER HAD A MECHANICAL FAILURE LEF TTING ALL AIR OUT OF SYSTEM, CAUSING THE BRAKES TO FAIL. WHEELS WERE NOT CHOCKED.
