While cleaning loose rock from around a cable spooling reel chain. EE pulled cable catching 2nd EE's finger on the chain sprocket.

While repairing a miner waterline rib rolled onto employee. Broken left lower leg & spinal cord injury.
While inch tramming RB on next row of bolts rock fell striking him in back of Rt leg.
EE was setting first belt in #6 Right when a piece of draw rock fell hitting him on his left shoulder.

While tightening bolts with hammer and wrench, hammer ricocheted off fender striking finger.

While driving a shuttle car he ran over something in the road that bounced him out of his seat, he then ran into rib and stopped he got off the SC. When he tried to stand up he developed a muscle spasm in his mid back.
Piece of rock fell from roof between the bolts and rib striking his left side.
