APPROX 12" ROCK JAMMED CONVEYOR BELT. EE TRIED TO DISLODGE ROCK WITHOUT TURNING OFF CONVEYOR. WHEN OBSTRUCTION WAS CLEARED, CONVEYOR BELT STARTED MOVING AGAIN, ROCK TILTED BACK AND CAUGHT HIS HAND BETWEEN CONVEYOR METAL GUARD AND ROCK, SMASHING TWO LITTLE FINGERS ON LEFT HAND.

A ROCK POPPED OUT OF THE JAW CRUSHER OVER THE GUARDRAIL, AND HIT HIS LEFT HAND, WHICH WAS RESTING ON THE RAIL. HAND HAD AN ABRASION AND A FRACTURE ON THE MIDDLE FINGER.

AS EE DROVE THE WATER TRUCK DOWN A SHORT STEEP HILL, WITHOUT GETTING THE TRANSMISSION IN GEAR, THE BRAKES FAILED. IN ORDER TO STOP THE TRUCK HE RAN INTO THE TIRE OF THE LOADER. EE WAS FOUND IN THE FLOORBOARD OF THE PASSINGERS SIDE OF THE CAB. HE WAS HELPED OUT OF THE CAB AND SAT ON THE GROUND. PRESSURE WAS APPLIED TO THE LACERATION OVER HIS RT EYE, TO STOP THE BLEEDING. EE WAS THEN
