Employee was training on a Cat 627G, first day, he was fully trained and was attempting his first pull when he was turning and hit a bump. The employee was "whipped" forward and seated hard. He has a compression fracture of his L2 vertebra in his lower back.

Employee was shutting door on the 966 Unloader (cat loader) employee's foot slipped and he fell off the unloader to the ground (about 8 ft) causing laceration to left knee, requiring stitches along with contusions & scrapes to left side of face.

WORKER AND ANOTHER EE WERE REPLACING THE CUTTING EDGE ON A SCRAPER PAN. IN DOING SO, ONE WORKER WAS HOLDING THE SCRAPER BLADE IN PLACE AND OTHER WAS REPLACING EDGE. INJURED WORKER EXPERIENCED PAIN IN GROIN AREA WHILE DOING THIS JOB.

WORKER WAS OPERATING 627 CAT SCRAPER PAN IN MINING AREA. WHILE TRAVELING OVER ROUGH TERRAIN, THE AIR RIDE SEAT FAILED TO WORK PROPERLY, THROWING HIM AGAINST THE ROOF CAUSING THE SAFETY RESTRAI NTS TO STRAIN HIS MIDSECTION. UPON INVESTIGATION IT WAS FOUND THAT THE SHOCK ON THE SEAT WAS BROKEN. WORKER SUFFERED A UNILATERAL INGUINAL HERNIA.

THE IMPACT OF EMPLOYEE AGAINS T HIS DOOR CAUSED INJURY TO HIS LEFT SHOULDER. NO SPECIFIC DIAGNOSIS WAS MADE. DIAGNOSTIC PROCEDURE WAS PERFORMED 10-14-93.
