EMPLOYEE WAS USING A BAR TO CLEAN OUT GRIZZLY AND ANOTHER EMPLOYEE HIT BAR WITH A SLEDGE HAMMER AND A SLIVER OF STEEL BROKE OFF AND FLEW INTO EMPLOYEES ARM.

EMPLOYEE WAS GETTING ON 1474RT DOZER AT THE START OF AFTERNOON SHIFT. HE PLACED HIS LEFT FOOT ON THE BOTTOM OF THE STEP AND LEFT HAND ON THE RAIL. HE THEN PLACED HIS RIGHT HAND ON THE RAIL AND PROCEEDED TO CLIMB ON TO THE DOZER. WHEN HE PUTHIS WEIGHT ON THE BOTTOM STEP, THE STEP BROKE, CAUSING A SUDDEN JOLT TO HIS RIGHT SHOULDER. SAW DOCTOR 11/19/93. BEGAN LOSING TIME 12/17/93.

INJURED WAS TRYING TO REMOVE A LOADER TOOTH WHICH HAD BECOME LODGED IN THE JAW ON THE PRIMARY CRUSHER. HE HIT THE TOOTH WITH A SLEDGE HAMMER WHEREUPON IT FLEW OUT OF THE JAW AND HIT HIM IN T HE FACE. DAMAGE TO THE FACIAL BONE STRUCTURE WAS SEVERE.
