Employee cut his left arm with a utility knife while scrapping paint off of an idler bracket. He sought medical attention, received stitches and returned to full duty the same day.

ON 3/25 EMPLOYEE WAS REMOVING A JAW CRUSHER WHEEL WITH A SLEDGE HAMMER WHEN HE FELT A PAIN IN HIS RIGHT SHOULDER. HE SOUGHT MEDICAL ATTENTION ON 3/28 AND WAS DIAGNOSED WITH A SHOULDER STRAIN. HE WAS PLACED ON RESTRICTED DUTY.

Employee experienced elbow pain after using a sledge hammer to drive bolts out of a crusher. He sought medical attention on 1/7/14, was placed on restricted duty for 3 days and returned to full duty on 1/13/14.

Employee was pouring concrete, attempting to move to another area of the pour, when his boot became stuck under the rebar causing him to trip. He stuck his hand out to break his fall and fractured his left middle finger. He is currently on restricted duty. This incident is being reported within 10 days of the employee seeking medical attention.

EE strained himself while he was lying on a creeper adjusting the front brakes on a forklift. The straining resulted in a hernia. This incident is being reported March 29, 2012 which is within ten days of seeking medical attention.

EE strained his back & right hip on 4/10/09 while using a pry bar to unhang material. He did not seek medical treatment until his initial doctor visit on July 2. EE was not placed on restricted duty, but was referred to a physical therapist for further treatment. This injury is being reported within 10 business days of the initial doctors visit on July 2, 2009.

Employee was walking down stairs leading from an area where he had been inspecting fire extinguishers when he missed the fourth step, from the bottom of the stairs and fell. He fell on his right ankle.

While employee was washing a weld, sparks sprayed onto his shirt from the torch and caught his shirt on fire. Employee stated he did not need medical treatment that he would be o.k. On 11/10/05 employee decided he would like to go to the doctor and get the burn checked on.

Employee received a foreign object in his left eye apparently from welding and grinding on a pump barge. He did not notice anything wrong until the next day and did not seek medical attention until 11-01-04. This is reportable because a prescription medication was used in his eye.

EE WAS LOADING A BACKHOE INTO A SHIPPING CONTAINER. WHILE USING A PAIR OF PULL JACKS TO PUT THE BACKHOE WHERE IT NEEDED TO BE, EE'S LEFT PINKIE FINGER WAS PULLED INTO THE SPROCKET OF THE PULL JACKS THUS REMOVING THE TIP OF THE FINGER AND MOST OF THE FINGERNAIL.
