EMPLOYEE WAS CL3MBING DOWN OFF OF THE MACHINE AFTER CHANGING THE SCREENS, SLIPPED AND FELL 2 1/2 TO 3 FEET ONTO THE CATWALK, HITTING HIS KNEE ON THE END OF A SPRAY BAR.
EMPLOYEE WAS CL9ANING MATERIAL OUT FROM UNDER THE CONVEYOR BELT, REACHED UP TO WIPE DIRT OFF OF HIS NECK. ARM WAS CAUGHT IN CONVEYOR BELT AND ROLLERS. RICHT SHOULDER WAS BURNED, RIGHT ARM MUSC LES PULLED AND 6RUISED.
