The employee had a pre existing shoulder injury and in the process of lifting a piece of metal in the shop they pulled and or strained their shoulder

Miner reported 11/07/2022 had a pain in groin area that went away 30 min later. 2 weeks later EE was lifting a clump of clay and felt a sharp pain again in groin and hurt to walk. 11/8/2022 we sent them to see a doctor and was diagnosed with a hernia and released to return to work with the restrictions.

A shop mechanic was grinding a small piece of metal on a bench grinder. The metal scraper blade EE was grinding was pulled from EE's hand between the grinder wheel and the guard, which caused EE's thumb to be pulled between the grinder wheel and guard. The mechanic suffered a cut on EE's finger and the tip of EE's thumb was removed.

Maintenance employee was changing brush on mobile sweeper. While picking up the brush they strained and felt some pain in groin area. This straining caused a hernia on the right side of groin area.

Operator was hooking up super sack with rubber strap. Pulled on rubber strap while stepping off of scale. Lost grip on rubber strap causing strap to re-coil striking operator in the right eye resulting scratches to eye, small cut above the eye.

Cleaning out extruder using a shovel. Shovel became lodged in sticky product. Employee went to pull shovel out and the shovel caught ring finger on right hand between shovel handle and die changer on extruder. X-rays showed that right ring finger was fractured.

Crusher cover for the Smooth roll crusher had been taken off the machine and placed on the floor. Employee was cleaning out the crusher from a ladder. Crusher cover plate was placed approximately 5 ft. from bottom rung of ladder. Employee came down the ladder and stepped backwards until coming in contact with crusher cover. Employee tripped and fell into the cover.
