LOWERING SCREENBOX ONTO NEW SUPPORT, COMEALONG SPRUNG WHILE BEING LOWERED BY MECHANIC AND HANDLE SPUN AROUND. HANDLE OF COMEALONG HIT LT. RING FINGER. NAIL WAS TORN, SIDE OF FINGER LACERATED AND FIRST DIGIT WAS SMASHED.
EE HAD BEEN FILLING UP THE WATER TRUCK FROM THE STAND TANK. HE FINISHED THIS TASK AND STEPPED UPINTO THE WATER TRUCK. HE TWISTED HIS TORSO AS HE WAS GETTING INTO THE DRIVER'S SEAT AND FELT LOW BACK PAIN.

HAD TO CHANGE SAND SCREEN ON SCREENING PLANT WHEN PUTTING IN NEW SCREENS YOU HAVE TO PUSH UP FROM UNDER SCREEN & PULL UP CLAMP BAR TO CATCH LIP ON SCREEN TOP SURFACE. THERE IS A VERY SMA LL SPACE TO WORK.

EMPLOYEE WAS PICKING UP HIS TOOLS OFF THE TAILGATE OF THE PICKUP. HE WENT TO CLOSE THE TAILGATE WITH THE TOOLS IN HIS RIGHT HAND. AS HE SHUT THE TAILGATE, THE RAZOR KNIFE HE WAS HOLDING CUT IN TO THE FLESHY PART OF HIS HAND BETWEEN HIS THUMBAND INDEX FINGER.
