EE HAD BEEN FILLING UP THE WATER TRUCK FROM THE STAND TANK. HE FINISHED THIS TASK AND STEPPED UPINTO THE WATER TRUCK. HE TWISTED HIS TORSO AS HE WAS GETTING INTO THE DRIVER'S SEAT AND FELT LOW BACK PAIN.

EMPLOYEE WAS PICKING UP HIS TOOLS OFF THE TAILGATE OF THE PICKUP. HE WENT TO CLOSE THE TAILGATE WITH THE TOOLS IN HIS RIGHT HAND. AS HE SHUT THE TAILGATE, THE RAZOR KNIFE HE WAS HOLDING CUT IN TO THE FLESHY PART OF HIS HAND BETWEEN HIS THUMBAND INDEX FINGER.
