After conveyor did not start, Employee went to check the reset button on the electrical panel. When they bent down to see it, their hand was close enough to the panel that they believes sweat dripped from their hand and electricity jumped from panel to their left pinky finger.

Carrying 4x4 oak blocks (one in each hand) to chock wheels of conveyor for maintenance. When setting down block in right hand, block fell from hand and twisted EE's right forearm. Diagnosis was strain of right bicep.

Shop technician working on crusher, #1 toggle seat in toggle plate. Toggle seat swung loose, pinching right index finger between seat and frame. Contusion to right index finger. No lost time. No restrictions.

EE did a lot of pulling, pushing & lifting during the week, helping to set up crushing spread at K-pit. On Friday the 18th, he had pain on left side of groin. He reported HR on Monday & had hernia diagnosed on Sept. 18. No treatment had been given, no lost time, no restrictions until surgery on November 7, 2008.
