EE reported he lost grip to ladder while descending down to bottom of bin area. After losing grip from ladder he slid down the ladder falling against bottom of bin and hitting his right ankle.

EMPLOYEE WAS LOADING A SHOT. HE PUT 4 BAGS IN THE HOLE AND THREW THE BAGS BEHIND HIM. HE WENT TO GET ANOTHER BAG WHEN HE STEPPED IN A HOLE COVERED UP BY AN EMPTY BAG. THE EMPLOYEE WAS TAKEN TO JEFFERSON MEMORIAL HOSPITAL WHERE A SPRAIN WAS REVEALED. EMMPLOYEE WAS GIVEN MEDICATION AND PUT ON RESTRICTED ACTIVITY.
EE alleges he slipped on the wet steps of the D-8R dozer twisting his left foot on 6/9/05, but did not report the accident to Employer until 6/16/05. Employee states he fell backward landing on his left foot.

EE ALLEGES HE WAS LIFTING 2 SHEETS OF METAL HE HAD JUST CUT UP ONTO THE WORK BENCH. HE THEN FELT A SHARP PAIN IN THE MIDDLE RIGHT SIDE OF HIS BACK.
