Two rock falls in one right off #2 main in work out panel one in #7 entry 5' off inby spad #481. Size of fall 8' high, 18' wide, 70' long. One in #4 entry. Fall start 50' outby spad #479 and goes 150' inby spad #479. Fall size 8' high, 200' long, 18' wide.
Employee was plastering a stopping and got plaster in his eyes.
Rock fall in return in #1 entry 2nd break on first left off #1 mains at spad #737. Not going to clean up fall.

When driving mantrip out hit small rock with wheel making steering wheel hit his right hand. He went to hospital. After he went home he called back and said it was just bruised.
A rock fall in old works fell at spad #498 and spad #501. Was cribbed, timbered and dangered off.
Top in break #35 inby spad #243 had been cribbed off for 3 weeks and dangered off. On 4-15 top fell in at break #35 about 40' long, 19' wide, 4' on left side and 6' to 7' on right side.
Operator had hand lying on leveling arm when he raised the head of the bolting machine & caught his finger between leveling arms.
