HE WAS OPERATING A 560 LOADER IN THE QUARRY LOADING THE QUARRY HAUL TRUCK. THE CONTINUOUS OPERATION OF THE LOADER CAUSED HIS SORE NECK. HE WENT TO THE DR. 5-29-97. HE HAS HAD PREVIOUS BACK INJ URY ON THE JOB SO THIS AGGREVATED THE SITUATION.

HE EXPERIENCED BACK PAIN WHILE DRIVING HIS EUCLID 122 DUMP TRUCK IN THE QUARRY SINCE THE BEGINNING OF THE MONTH. HIS SEAT HAS NO BOUNCE TO IT SO HE FELT EVERY BUMP. THE BACK PAIN HAS CONTINUED OFF AND ON UNTIL YESTERDAY WHEN HE BENT OVER TOPICK UP A TONE AND IT GAVE HIM INTENSE PAIN.

ON THRUS AND FRI HE NOTICED A LOT OF CHEST PAI WHILE HE WAS SHOVELING. ON MON 2-12-96 HE REPORTED TO WORK BUT WAS IN SEVERE PAIN AND HAD TROUBLE BREATHING.

HE WAS POUNDING A WEDGE INTO A BLOCK OF STONE. HE SWUNG THE SLEDGE HAMMER AND HIT THE FORK ON ALOADER AND THE SLEDGE HAMMER BOUNCED OFF THE FORK AND HIT HIS FOOT. HE SAW A DR. AT THE CLINIC W HO SENT HIM TO SEE A SPECIALIST, TOOK X RAYS ANDDETERMINED HE HAD A FRACTURED TOE. HE WILL BE OFF OF WORK UNTIL HE SEEN BY THE DR. AGAIN IN TWO WEEKS.

THE NEXT MORNING SAW HIS OWN DR WHO SENT HIM TO A SP[ECIALIST. SPECIALISTS SAID HE HAS A SEVERE CONTUSION AND HE WIL BE OFF WORK A LEAST THRU JULY 6 1994. GIVEN MEDICATION.

THE INDIVIDUAL WAS GETTING PAINS IN HIS ARM AROUND THE 20TH OF JUNE. HE TOLD HIS FOREMAN BECAUSE THE PAINS WERE GETTING MORE SEVERE. HE WAS ADVISED TO SEEK MEDICAL TREATMENT WHEN THE PAINS WER E CONSTANT AROUND THE 15TH OF AUGUST. HE SAW THE DOCTOR AND WAS TOLD HE HAD A HERNIA AND NEEDED SURGERY. HE RECEIVED THESE PAINS DUE TO YANKING AND PULLING ON THE ARM OF THE BIN TO EMPTY STON
HE HURT HIS BACK AS HE WAS LIFTING APIECE OF FLAGSTON.E HE HAD A SHARP PAIN IN HIS BACK AND COULDN'T STAND UP. HE REMAINED BENT OVER, MOVED TO THE SCALE AND SAT DOWN UNTIL HE COULD STRAIGHTE N UP AGAIN. HE WAS TAKEN TO THE CLINIC TO SEE A DR. DR SAID HE STRAINED A MUSCLE IN HIS BACK. TOLD HIM TO TAKE A DAY OFF WORK AND THEN RETURN.

WASTE STONE IS THROWN OUT THE DOOR OF THE BUILDING TO THIS AREAANOTHER EMPLOYEE DIDN'T SEE HIM AND THREW A PIEC E OF STONE OUT THE DOOR. HE WAS HIT IN THE HEAD CAUSING A CUT THAT REQUIRED 27 STITCHES AND HE HAD A MILD CONCUSSION.
