Green Section 30 LLC · Struck by falling object

Plant Operator was walking around crusher during operation when EE went to the control panel and removed hard-hat and walked underneath a conveyor where a rock bounced off of the belt and fell approximately 8 feet and struck the miner in the head. The miner felt dizzy and nauseous then went to the hospital for further evaluation.

At approx 10:30 AM EE was operating Cat D8R dozer. EE proceeded to dismount the equipment to visually examine work area. EE's foot was caught between blade rise & machine tracks while simultaneously slipping on the wet & muddy tracks. This caused EE's left ankle to twist & then caused EE to fall onto tracks. EE called Pit Foreman at 10:47 AM & notified Foreman

Labor was tightening bolts for transfer conveyor assembly, with a wrench using ee whole body weight. The wrench slipped of the bolt and came back which struck the lower right side of ee head.

Hired mechanic was using porta power to push out the tail pulley of the impactor. The wood snapped and sent the tail pulley back and EE's hand got stuck/crushed in the impactor. The machine was reset and the mechanic went to the hospital.

Performing maintenance on crusher on rainy day. Slipped while climbing ladder to check fluid levels. Fell 4-6 ft. to ground, landing flat on back. Back tightened up over weekend, went to doctor on 10/02/2017.

Miner was standing on the third rung of a step ladder performing maintenence on a screening plant when miner lost his balance and fell backwards onto his left side in the dirt and mud. Miner suffered a concusion, seperated left shoulder, strained neck, strained back and ligament damage to his right knee.

Miner was checking on a screening plant, the upper deck had gotten clogged with debris, before the miner was able to shut down the machine a 4" rock came off the upper deck of the screening plant striking the miner in the side of the face. The miner sustained a contusion and laceration requiring 3 stitches.

Miner was walking on the catwalk of the main screen plant when he contacted an electrical line, (which had some insulation rubbed off due to placing a screen up against the line) the line arced on the hand rail and shocked the miner, causing him to fall against the hand rail straining his back.

Miner was assisting mechanic with routine maintenance of rotating cutting edge on a John Deere 824 loader bucket. While the mechanic and miner were rotating the cutting edge it slipped out of the mechanics hands and the miner was unable to maintain his grip on the other end dropping end of cutting edge on his foot, fracturing his right #1 toe.

Miner had placed socket with rachet on bolt to tighten jaw crusher adjustment; spring tension was released pushing socket and rachet toward steel I-beam. Miner's hand was on rachet head instead of handle of rachet, crushing finger between rachet and steel I-beam lacerating finger requiring stitches.
