An employee was drilling into a piece of metal when the drill bit got stuck. When this occurred they lost grip of the drill. When this occurred it went around and hit them in the back of the hand. This resulted in a fracture of a finger. As of 05/09/2023 change of status.

Haul truck operator contacted the shift foreman and reported illness in the form of chest palpitation's, and numbness of the hands. The foreman called a mayday and ERT responded, obtained vitals and made the decision to contact local EMS. Operator was flown to NNRH were they were evaluated and release with a diagnosis of dehydration, non-work related.

While operating an impact gun to remove bolts in a tight spot, the bolt broke free and the mechanic began pulling the impact gun out of the spot and caught thumb on a metal burr causing a cut requiring one stich on left thumb. The mechanic was wearing gloves.

At approximately 11:30 haul truck 352 was being loaded in the Pick Pit. Upon being loaded with material the truck rocked side to side. When the truck rocked the operator experienced pain in the left back area. Operator parked the haul truck and notified the foreman.

Employee was wearing a good traction boot.

An employee was sitting in the buddy seat of a Cat 777 haul truck while training a new employee. The haul truck driver reversed under the bucket of a 993K loader. A tooth on the bucket of the loader contacted the haul truck, causing it to rock. This motion aggravated the trainer's back.

At 0300 A.M I was swinging the dead blow to remove a power cable when I'm in the middle of my fourth swing I felt my middle back tense up and my knees buckled unable to fully stand back up. I proceeded to finish the shift.

The miner was on an A frame ladder, the ladder twisted and the spreader broke which resulted in the miner falling and spraining EE's left ankle. Miner refused medical transport and continued EE's shift. On Thursday 3/5/2020 employee got the ankle looked at by a doctor. When ankle was x-rayed it was diagnosed with a small hairline fracture to the fibula.

While trying to pull a heavy pallet out of the back of a pickup, the employee felt a 'pop' in the left forearm and immediate pain. The resulting injury was a torn ligament from the bicep that separated from the forearm commonly known as a "Popeye" injury.

IE and co-worker drove down and immediately reported the incident and cold pack was introduced to this strained area of the arm.

Cat 777D truck driver was traveling uphill loaded to dump when they came to a stop & rolled back down the road traveling approximately 300' before going up an embankment & rolling over one complete turn, coming to a stop facing uphill on its wheels in the downhill travel lane. Victim was found shortly after not breathing belted into seat. Subsequent efforts could not revive them.

Technician manually lifted two turbo cores (weighing about 60lbs ea.) into service truck. They placed a box on each of them to minimize lifting, then rolled the boxes into place for transport. When rolling, they felt a minor pain in their back that worsened over time. That day the temperature reached a high of 42 degrees with heavy snow flurries and wet, muddy walking surfaces.

Employee was attempting to clear ice and snow from the containment area. While descending into the containment, employee's footing slipped on the last (3rd) step and employee fell. Employee's left ankle was fractured as a result of the fall.

The employee was working on roof purlins, installing the insulation and roof panel when Ee slipped and fell through the insulation to the interior of the building. The cable of the retractable Ee was using snapped and Ee fell roughly 20' to the concrete floor below. We have been told Ee has 4 broken ribs and 3 fractured vertebrata.

Employee was cutting geosynthetic material toward the body and blade cut through glove into left hand. Our customer's site safety coordinator took employee to the closest medical facility for evaluation. Laceration determined first aid. Proper cutting techniques were covered during orientation and safety training (OSHA 10) and were not followed in this situation.

The injured employee climbed up the concrete forms to handle the concrete pump hose. When the pump was shut off the hose kicked causing the employee to fall and fracture clavicle.
