CRG Mining LLC · Over-exertion (Not Elsewhere Classified)

The miner was preparing to rock bolt heading after taking a normal round. When EE began to maneuver the jack leg, EE popped their right shoulder. EE's heading was clean and there were not any tripping hazards.

CRG Mining LLC · Struck by rolling or sliding object

The lead miner was bolting the face heading. The engineer and miner had both scaled the heading prior to the event. While bolting, a rock slid from the rib cutting through the miners boot. The injury broke EE's second to smallest toe and EE had a laceration on the foot. The shift supervisor investigated the heading and found that the rock became dislodged during bolting.

CRG Mining LLC · Fall to the walkway or working surface

The employee was walking to the portal and slipped on snowpack and ice landing on EE's left shoulder. The employee felt that EE was fine and we did not seek medical attention. The employee finished the 3 days left in EE's rotation. I checked in with the employee and EE stated EE was still feeling sore after 7 days off. I instructed EE that EE needed to see a doctor.

A supplier from Munro Supply had dropped of some supplies in the office. They suffered a major heart attack and passed away. They were later found by a employee and CPR and AED were administered and 911 was called.
