Driving a loaded HT up the ramp, when employee encountered a jeep coming down the ramp. Stopped and began backing up to make room for jeep to pull into muck bay. Employee stated: tried to stop and move forward but continued to go backwards down the ramp. HT glanced off rib and ran into the front of a 2yd LHD coming up the ramp approx. 200 ft. away. HT rolled over on its side.

While tramming 2yd LHD out of the mine, employee got to the spiral between the 650 and 500 level. As employee came around the corner, saw the 18 ton haul truck rolling backwards at an unusual rate of speed. Attempted to reverse LHD and braced for impact. Haul truck hit LHD at the bucket end and bounced it off the rib. Employee was wearing EE's seatbelt.
IE was cleaning out drill holes with air using a blowpipe and hose when EE felt a pain in EE's lower stomach area. IE sustained a hernia.
While grabbing and picking up a jackleg from the bolt deck and transporting it approx. 12ft. then placing against the rib to secure, employee felt a cramp like pain in right bicep area.
While operating a Jackleg drill, the drill steel shanked causing the drill to drop which pulling EE's left arm when it dropped. The employee was evaluated and release to EE's normal duties. EE later experience difficulties with EE's arm and will be scheduled for surgical repair to tendon tear on July 15, 2020 (notified the mine on 07/13/2020)

Miner was operating a Model DT-12 dump truck, while backing up miner placed hand on the outside of the ROPS post and while articulating the truck around the corner, miner's left middle finger was pinched between the ROPS post and the bed of the truck resulting in required sutures to close the wound. No bones were broken.
Bolting with jackleg, just about to complete the hole, looked up to see how much was left to drill and a piece of rock from the back came down and struck them in the lower right cheek area resulting in a broken tooth and small laceration on lower lip.

Miner was shotcreteing and twisted right knee from stepping on a rock.
Miner was repositioning a 75 horse power fan in the loader bucket. The fan rolled and smashed left knee into loader bucket, pinning knee against the inside of the loader bucket. Causing bruising and swelling of left knee.
Miner was descending a ladder from mantrip, when he went to step on the ladder it slipped out from underneath him causing miner to fall and hit end of mantrip, resulting in contusions to left rib.
Operator was unloading an empty barrel off of forklift, stepped on a mill liner and twisted right knee.
Miner was bolting with a Jack Leg while collaring a hole. A rock came down and hit his right forearm causing laceration to right forearm, resulting in staples.
LHD bucket tilt cylinder hose failed at fitting. Hydraulic oil came in contact with miner resulting in 2nd and 3rd degree burns to left hand, arm and neck. Minor burns to right forearm.
While installing ground support in the back, under supported ground. The miner lost control of his Jackleg, knocking the miner to the ground. The miner received a laceration to his left shin requiring four staples. The miner was treated and given a full release.

Drilling face with Jackleg, went to move machine, the leg was pressurized causing the leg to extend thus pulling right shoulder out.
Mechanic was changing a bearing on the tail pulley when a chain slipped and hit him in the mouth causing two lacerations to his upper lip requiring sutures. Mechanic was treated and received full release for his next scheduled shift.
Miner was installing a roof-bolt in the back when a small rock came loose causing a laceration on the left forearm requiring stitches, the miner returned to work the same shift with a full release.
Mechanic was rebuilding a differential 3rd member on a metal work table. While attaching the drive yoke the 3rd member rotated, the mechanic reached to stabilize the 3rd member straining right elbow. A follow up Doctor visit resulted in work restrictions effective 02/19/2013

Miner's right foot pinched by 21B EMICO Mucker. Miner was wearing metatarsal boots. Miner was treated for first aid at Valley View Medical Center.
Miner washing equipment part in parts washer. Part slipped out of hand smashing finger. Stitches required.
Operator was cleaning the grizzley at the crusher, backed up and lost his footing. Operator reached for gate with right arm to stop from falling, resulting in right shoulder dislocation. Operator was on restricted duty since 5/14/12. He had surgery on 6/7/13 resulting in a lost time accident.
Miner was collaring hole looked up and small rock aprox 2in fell striking miner in the mouth. Miner was wearing appropriate PPE, area was clear of visible loose rock, area was bolted from the back to the face visible scaling had been done , housekeeping was good.
Miner complained of knee pain, it was found out that he had been having knee pain for a while. During investigation it was determined the area was free of loose rock and had a level walking surface. Miner was walking to the buggy with normal walking patterns when he went to take a step on his left foot and his knee popped out aggravating a non traumatic undetermined injury.
Miner was bolting when small rock came down and hit his left ring finger. Miners 5-point card shows that he had scaled the area prior to performing work.

While changing out the long hole steel Miner grabbed a-hold of the steel while driller was rotating to remove it. The Steel became bound up at an angle and pinched his thumb between the steel and the guide slide.
While bolting barred down some loose rock and continued to bolt.Slipped on a slab that had been barred down and landed on my right elbow.
Employees was unloading bolting supplies underground ( mats ) when one that a sharp edge cut through his glove he then received a laceration. Employee was wearing all PPE's at the time
Miner was driving a bolt with 1 1/2 foot driver he got the bolt tight to the back and the machine would not come down. EE went to move the machine leg forward to remove pressure off the driver and when doing so the machine came down in EE had his foot under the crows foot, the weight of the machine pushed the crows foot through EE's met-guard boot and into his foot.
Miner was using a cutting torch when a piece of slag came off and went behind his safety glasses burning corner of left eye.
Mechanic was installing a hoist cylinder by hand when he went to lift it he felt a sharp pain in his left bicep. Per another EE there is a suspicion on steroid use from this employee. Employee did not report this incident until 5:45pm
Miner was cleaning out a conex and was pushing and pulling a cylinder aprox 200lbs, Miner has had a hernia before and failed to use S.L.A.M and correct lifting techniques.
Miner was moving a ladder underground ladder became caught on a rung and miner went to lift it out during the process of him lifting the ladder he did not use proper lifting techniques causing him to strain a muscle in his back.
Miner 3 was chucking steel with a Miner 1 when the steel shanked and Miner 1 operating jack leg lost control. Miner 3 was struck in the jaw and cracked two teeth. Small scratches on jaw.
Moving jack-leg drill from one area to another, crows foot got caught on nine wire that was used to hold jack-leg to rib, lost grip on jack-leg and grabbed a hold of muffler to keep it from falling when his finger got caught in the pinch point. Lacerated left ring finger requiring stitches.
Miner was drilling the last knee hole in preparation for loading a round, when the air spud on the jack leg came loose and hit him in the knee. The whip check was properly installed on air line. Miner was on one knee next to connection. A deep contusion was the result of spud hitting Miners knee.
Drove mucker bucket down to the work area. Miner ran over a big rock by first muck bay. After he parked and got off mucker the miner bent down to pick up water hose he felt a shooting pain down his lower back then down his left butt cheek.
While bolting the back, miners drill bit came off the end of his steel and landed in a puddle beyond the last row of bolts. The miner went past the last row of bolts to retrieve the drill bit and was hit with rock that fell out of the back.
Miner was walking across travel way towards office when she tripped over a small rock. She hit and lacerated her left knee.
While maneuvering a toggle plate into place the miner felt a pain in his back and when he stood up his right testicle had a sharp pain.
Employee did brake check on surface, stoppped twice on ramp with no problems, when got to the 650 ramp brakes failed and miner hit rib to stop. Miner was not wearing seat belt and was thrown around in cab.

While removing the cribbing from under a haul truck the mechanic was stacking the wood on the side when he smashed his right hand ring and middle finger between two pieces of cribbing.
While taking down a welded wire bulk head miner leaned ladder on bulk head and gut the chain holding it up. The bulk head fell and the miner received cuts and punctures to the left arm.
EE was in the process of removing a wooden ladder from the top-mounted ladder truck of a pick up truck. EE stepped off of the running board to the ground (approx. 12") and twisted her right ankle.
Miner was stepping off of mantrip using three point contact and while planting his right leg, EE aggravated an old injury by twisting his knee.
While drilling uppers in the 220 stope, miner looked up at his drill steel and hole when a rock fell from the drill hole area and struck him in the nose and mouth.
While loading jack leg drills into a mucker bucket for removal from area the drill he was handling slipped and pinched his pinkey finger of the left hand against the bucket lip. EE pulled his hand back and ripped the tip of his pinkey off into his glove.
Put split set in one hole of the roof mat. Scaling bar that was holding mat up slipped and mat hit a rock that dislodged it. Rock fell down hitting miner in the lip and arm causing a laceration on both.
While doing ground control with a jack leg, the steel became plugged while removing the steel from hole it got stuck in. Grabbed the jack leg and when the jack leg came loose, employees finger was in a pinch point, required stitches.
While driving a haul truck the front of the box hit a 6" pipe at the vic coupler and seperated it. While exiting the vehicle miner received a laceration on his head and abrasions on his arms.
EE was hanging welded wire to shot crete the back for ground control when a sharp rock fell thru the screen striking EE on the right cheek at the corner of his mouth. EE stated he was fine and stopped the bleeding with a butterfly band-aid. After EE's shift He went to the local Urgent Care for evaluation and received 2 stitches.

WHILE DRILLING OUT A FACE ROUND IN 5007 RAMP, HE STARTED TO COLLAR A HOLE WHEN A ROCK CHIP FLEW BACK AND STRUCK HIS RIGHT EYE. THE DR. REMOVED THE CHIP BY USING A Q-TIP AND GAVE HIM A PRESCRIP TION FOR EYE WASH.
WHILE HOLDING AN 8' LONG WOODEN POLE IN A HOLE UP RIGHT SO IT COULD BE BACK FILLED WITH A TRACK HOE BUCKET, THE HOE BUCKET HUNG UP ON A ROCK AND WHEN THE OPERATOR GAVE THE BUCKET CONTROL MORE FEED, IT BROKE LOOSE, HIT THE POLE WHICH HIT EE IN THE HEAD AND SHOUDLER. HE FLEW APPROX. 10' BAKCWARDS LANDING ON HIS BACK AND HIT HIS HEAD ON A ROCK CAUSING LACERATIONS AND A FRACTURE TO HIS
WHILE DRIVING THE KUBOTA DOWN THE RAMP AND INTO A SHARP, STEEP CORNER OF 5007 X-CUT, SHE TRIED TO AVOID HITTING THE BACK OF A HAULAGE TRUCK THAT WAS PARKED ON THE CORNER AND RAN THE KUBOTA INT O THE RIGHT RIB. THE TRACTOR TIPPED OVER HITTINGAND ENTRAPPING HER FOOT UNDER THE PASSENGER SEAT SIDE RAIL. SHE RECEIVED A HAIRLINE FRACTURE TO HER FOOT.
WHEN HE PUT A PIPE WRENCH ON ONE JOINT OF D.DRILL ROD TO HOLD IT WHILE THE DRILL TURNED THE OTHER ROD TO TAKE THE JOINT APART, THE WRENCH SLIPPED FROM HIS HAND, SPUN AROUND AND STRUCK HIS FING ER BETWEEN THE WRENCH AND THE DRILL. HIS FINGER WAS LACERATED AND NEEDED SUTURES.
MECHANIC TOOK THE RADIATOR CAP OFF TO CHECK THE COOLANT LEVEL. THE COOLANT WAS VERY HOT & UNDER PRESSURE & WHEN HE TOOK THE CAP OFF, THE HOT LIQUID BLEW OUT HITTING & BURNING HIS HANDS, CAUSIN G 2ND DEGREE BURNS
WHILE DRILLING INTO THE BACK TO INSTALL MATS & SPLIT BOLTS, THE STEEL KNOCKED A ROCK LOOSE, CAUSING TI TO FALL DOWN & BOUNCE OFF THE DRILL STEEL AND HIT THE MINER IN HIS NOSE. THE ROCK CAUSED A LACERATION THAT NEEDED SUTURES
WHILE ATTEMPTING TO RE-ENTER 5157 STOPE AFTER THE BLAST, HE CLIMBER UP THROUGH THE MANWAY OPENING & SLIPPED, FELL BACK & HIT HIS BACK AGAINST A 3" X 12" X 8' LAGGING THAT WAS COVERING THE TOP OF THE MANWAY. CAUSING SEVERE CONTUSION TO HIS BACK.
WHILE RIDING AS A PASSENGER ON A KUBOTA TRACTOR THE OPERATOR DROVE OVER A ROUGH SPOT IN THE ROADWAY CAUSING HIS PASSENGER TO BOUNCE UP & COME DOWN HARD ON A HARD METAL SEAT, JARRING HIS LOWER BACK. THIS CAUSED SEVERE MUSCLE STRAIN AND PAIN IN LOWER BACK.

WHILE BOLTING UO THE BACK & PUSHING THE MAT TO THE BACK, A SMALL ROCK LESS THAN 2X2 FELL FROM THE BACK, HIT THE RUBBER MUFFLER ON THE JACKLEG AND BOUNCED BACK AND HIT THE MINER IN HIS MOUTH CH IPPING HIS FRONT TEETH REQUIRING DENTAL CARE.
WHILE GETTING READY TO USE A CUTTING TORHC, THE FABRICATOR TOOK OFF HIS SAFETY GLASSES AND PUT ON HIS CUTTING GOGGLES AND GOT A PIECE OF SLAG IN HIS EYE. HIS GOGGLES WERE LAYING UPRIGHT AND H E FAILED TO CLEAN THEM OUT.
WHILE DRILLING THE BACK IN 5875 STOPE, THE MINER WAS CHANGING OUT HIS DRILL STEEL FROM AN 8' STEEL TO A 10' STEEL WITH ONE HAND WHILE TRING TO HOLD AND MAINTAIN THE BALANCE OF THE JACKLET WITH HIS OTHER HAND, WHEN HE TWISTED AND STRAINED HIS BACK.
WHILE BOLTING UP A CAP TO SECURE & LACE OFF AN OPEN HOLE. SOME ROCK FELL FROM ABOVE OUT OF THE HOLE & STRUCK THE MINER ON THE LEFT ANKLE. THE ROCK BOUNCED OFF THE MUCK PILE.
WHILE DRIVING DOWN THE RAMP, TRACTOR GAINED TOO MUCH SPEED AND STARTED BOUNCING. THIS CAUSED THE OPERATOR TO RECEIVE WHIPLASH TO HIS NECK.
WHILE MOVING HIS JACKLEG DRILL FROM ONE LOCATION TO ANOTHER [APPROX 2'] HE LOST HIS BALANCE & FELL. THE JACKLEG DRILL THEN FELL DOWN ON HIS LEFT LEG AT THE KNEE, BRUISING HIS LEG.
AFTER 3 TRUCKS OF MUCK WERE PULLED FROM THE STOPE, THE MINER WAS STANDING ON THE EDGE OF THE MUCK PILE WHEN IT GAVE WAY TAKING HIM WITH IT. THE MUCK SLIDE ENTRAPPED THE MINER UP TO HIS SHOULDE RS.
WHEN CLIMBING UP A MUCK PILE WITH A LADDER, HE STEPPED OFF THE LADDER AT THE TOP AND JAMMED HIS HEAD HARD INTO A 3 X12 STULL THAT WAS ACROSS THE STOPE FROM THE FOOT WALL TO THE HANGING WALL. T HIS STULL WAS PUT IN TO HELP EXTRACT AN ENTRAPPED MINER.
WHILE LAYING ON HIS BACK UNDER A LHD TO CHANGE OUT A DRIVE LINE, HIS RATCHET WRENCH KNOCKED SOME RUST AND LOOSE DIRT DOWN INTO HIS LEFT EYE. THE RUST SLIVER IMBEDDED IN HIS EYE.
WHEN PULLING A JACKLEG OUT OF A LIFTER HOLE AT THE FACE, THE LEG ON THE DRILL SLIPPED CAUSING THE MINER TO FALL BACKWARDS. HE STEPPED ON A ROCK, BENDING HIS ANKLE INWARDS, CAUSING [SPRAINED AN KLE].
WHILE CLEANING UP THE 500 MAIN RAMP BY BLADING THE LOOSE MATERIAL OUT FROM UNDER THE PIPELINE SO IT COULD BE PICKED UP WITH A LOADER, HE RAN OVER A LARGER ROCK WITH THE DOZER AND STRAINED HIS NECK (WHIPLASH) WHEN THE DOZER RAN OFF THE ROCK.THIS CAUSED A SWOLLEN NECK DISC.
WHILE UNLOADING A 55 GALLON BARREL INTO A LOADER BUCKET, HIS LEFT RING FINGER GOT SMASHED BETWEEN THE BARREL AND THE LOADER BUCKET CAUSING A LACERATION TO THE FINGER WHICH NEEDED SUTURES.
WHILE LOADING A 3" X 12" X 20' TIMBER ONTO A TRUCK. HE STEPPED ON A ROCK CAUSING HIM TO LOOSE HIS BALANCE. WHILE TRYING TO KEEP FROM FALLING DOWN HE STRAINED HIS LOWER BACK.
WHILE ATTEMPTING TO OPEN UP THE TOP OF THE RAISETO ENTER A STOPE AFTER DAY SHIFT BLASTED, THE MINER ENCOUNTERED SMOKE & GAS AND GOT ON OVEREXPOSER TO CO GAS.
WHILE BOLTING GROUND SUPPORT MATS, A SMALL ROCK FELL FROM BETWEEN THE MATS STRIKING THE MINER IN THE RIGHT WRIST.
WHILE THE MINER WAS DRILLING, DRILLHUNG UP CAUSING IT TO SPIN. HOSES STARTED WRAPPING AROUND THE MACHINE. MINER TRIED TO TURN OFF THE AIR AT OILER AND HOSE AND WHIP CHECK CAUGHT HIS ARM FRACTU RING HIS FOREARM.
WHILE DRIVING UP THE MAIN RAMP, THE NATURAL VENTILATION BLEW A FOREIGN BODY (RUST) INTO THE MINER'S EYE. A DR HAD TO DIG THE RUST OUT WITH A NEEDLE.
WHILE UNLOADING MATERIAL OUT OF A TIMBER SKIP AT THE BOTTOM OF THE RAISE, TIMBER SLIDE, A ROCK FELL FROM THE RAISE HITTING THE MINER ON THE BACK/TOP OF HIS HEAD CAUSING A CONTUSION AND LACERAT ION NEEDING SUTURES.
EE WAS DRIVING A HAUL TRUCK DOWN THE DECLINE TO THE 500 LEVEL AND HIT A BUMP CAUSING HER TO JAR HER BACK. BRUISED DISC.
WHILE TAKING THE DRILL RODS APART, THE ROD JOINTSPLIT CAUSING ONE OF THE ROD ENDS TO SPIN AROUNDAND HIT THE DRILLER IN THE TEMPLE
MINER WAS LIFTING A BUNDLE OF MATS AT ONE END, WHEN HE SAT THE END DOWN, HE SAT IT ON A ROCK & HIS FINGER WAS BETWEEN THE MATS & THE ROCK CAUSING A FINGER NAIL TO BE RIPPED OFF.
WHILE PICKING A HITCH WITH A PICK, A PIECE OF ROCK OR STEEL FLEW AND HIT THE MINER IN THE LEFT WRIST MAKING A PUNCTURE WOUND.
