EMPLOYEE WAS CLIMBING DOWN OFF ORE PILE AND THE GROUND COLLAPSED UNDERNEATH HIM. EMPLOYEE'S LEFT KNEE SLIPPED BETWEEN TWO ROCKS, CAUSING IT TO TWIST. THE ORE PILE WAS UNUSUALLY STEEP, DUE TO B LASTING AND SLOUGHING CONDITIONS.

EMPLOYEE WAS UNDERNEATH 235 CAT EXCAVATOR EXAMINING STARTER CONNECTION PRIOR TO STARTUP. HE CAUGHT HIS TAILBONE ON THE HOE HOOK UNDERNEATH AND SLIPPED TO THE GROUND FRACTURING THE TAILBONE.
