EMPLOYEE WAS SPLITTING SANDSTONE WITH A CHISEL & AN 8LB. HAMMER. IN THE SPLITTING PROCESS, HE MISSED THE CHISEL, STRIKING THE MIDDLE FINGER OF HIS LT. HAND. INJURY OCCURED ON 8/31/04 BUT NOT REPORTED UNTIL 9/2/04. EE WAS TREATED FOR INFECTION & CONTUSION OF THE INJURED FINGER, (2) LOST WORKDAYS.

WHILE INSPECTING A PILE OF STONE FOR QUALITY, EE STEPPED ON A PIECE OF STONE WHICH FLIPPED ON ITS SIDE CAUSING HIM TO TWIST HIS RIGHT FOOT. THIS ACTION CAUSED PAIN THEN NUMBNESS TO THE BIG TO E. DIAGNOSIS: CONTUSION TO RIGHT FOOT. EE RETURNED TO HIS REGULAR DUTIES 12/3/03.
