EE was manually placing pieces of sandstone onto a wooden pallet when he sustained a sharp pain to his lower back. Physician diagnosis: Lumbar strain.

EE was palletizing pieces of patio stone when he sustained a sharp pain in his lower back. Physician deignosis: lumbar strain He lost 1 day of work on 12/14/05. He returned to work on light duty on 12-15-05 until next evaluation of 12/7/05.

EMPLOYEE WAS SPLITTING SANDSTONE WITH A CHISEL & AN 8LB. HAMMER. IN THE SPLITTING PROCESS, HE MISSED THE CHISEL, STRIKING THE MIDDLE FINGER OF HIS LT. HAND. INJURY OCCURED ON 8/31/04 BUT NOT REPORTED UNTIL 9/2/04. EE WAS TREATED FOR INFECTION & CONTUSION OF THE INJURED FINGER, (2) LOST WORKDAYS.

WHILE INSPECTING A PILE OF STONE FOR QUALITY, EE STEPPED ON A PIECE OF STONE WHICH FLIPPED ON ITS SIDE CAUSING HIM TO TWIST HIS RIGHT FOOT. THIS ACTION CAUSED PAIN THEN NUMBNESS TO THE BIG TO E. DIAGNOSIS: CONTUSION TO RIGHT FOOT. EE RETURNED TO HIS REGULAR DUTIES 12/3/03.
