Employee was moving parts and a bee stung EE near EE's eye. No nest found, no other bees in area.

EE was busy both loading trucks and adding sand to the portable screening plant. EE had to move around the pit at a quick pace. EE was traveling across the pit and passed over the drainage ditch and EE did not see hole in pit floor. Passing over the hole caused the loader to bounce. The sudden bounce caused his head to move from side to side in a sudden fashion. EE on light duty

Independent contractor was on site constructing a covered fuel deposit. The final stages of the project were being completed when the carpenter lost his balance while removing some temp. braces and fell off of a step ladder. The carpenter fell onto his side as he landed onto the slab.

Incorrect us of pads to outriggers on lift truck. Ground shifted beneath outrigger, which caused load that was in the lift positon to swing. As the load began to swing, the equilibrium of the truck was lost and the truck tipped over onto the conveyor.
