Employee was shoveling dirt near kiln area during strong winds. Wind blew material back into employee's face and into right eye. Material could not be effectively removed on site and employee visited doctor. Doctor was able to remove material. Resulted in scratched cornea in right eye.

Noise induced hearing loss. Grefco has an HCP established for all employees & we are in compliance with MSHA regulations. We have audio metric testing done annually. Employee has been trained & has always worn hearing protection. He is currently & has in the past used plugs & ear muffs.

Went to unplug #3 screw w/air hose, was holding himself up w/left hand, operating air hose w/right. Slipped & saw his hand slip into screw, at the same time I pulled the trip cord but not quick enough. When he pulled his hand out I realized my fingertip was missing. The screw was guarded & had trip wire, small opening for maintenance only, to view for proper operation.
