EMPLOYEE WAS OPERATING HEAVY EQUIPMENT. AFTER 8 MONTHS EMPLOYEE COMPLAINED ABOUT NECK PAIN. AFTER THE COMPLAINTS, HE WAS MOVED TO CRUSHER OPERATOR POSITION; A SITTING POSITION. EMPLOYEE LAID O FF DUE TO LACK OF WORK. EMPLOYEE INFORMED COMPANY ABOUT SURGERY. EMPLOYEE WAS NOT SURE HOW INJURY WAS SUSTAINED,BUT A CABLE BROKE AND STRUCK HIMON SHOULDER. NO FIRST REPORT WAS TURNED IN.

EMPLOYEE HAS COME UP ON THE PORCH AND SET HIS LUNCH BOX DOWN PREPARING TO UNLOCK THE DOOR. AFTERUNLOCKING THE DOOR HE TOOK A STEP BACKWARD AND TRIPPED OVER HIS LUNCH BOX AND FELL OFF OF THE PO RCH ONTO THE GROUND FLAT ON HIS BACK.

EMPLOYEE WAS TRYING TO REPLACE THE PIN IN THE TOOTH OF THE LOADER BUCKET. HE COULD NOT GET THE TOOTH IN THE PROPER POSITION SO HE HIT IT WITH THE HAMMER TO ACHIEVE THE PROPER ALIGNMENT. WHEN H E STRUCK THE TOOTH A SLIVER OF METAL CAME OFF OFTHE TOOTH AND RICOCHETED INTO HIS UPPER LEFT CHEST.
