Mountain Valley Stone Inc · Fall from ladders

Miner was using a chair to reach an item on an upper shelf. The chair slipped out from under the miner causing EE to fall. The miner fell directly on EE's left wrist. Resulting in a fracture of EE's ulna and radius bones
