Hageman Plant Metal/Non-Metal

Employee was walking up the steps of our main plant for maintenance to hook up a chain when they slipped and fell. Employee hit their right hand during the fall.

Fueling a piece of equipment & caught self with left hand/forearm & right hand when EE started to fall off the equipment step when reaching for the fuel tank step. Initial doctor's assessment was that it was a left shoulder sprain. Further assessment on 10/11/22 indicated surgery was necessary. EE will work at full capacity up until surgery on 10/18/22.

Employee tripped on the concrete ground while getting rubber from under the shelves, injuring the right knee. Employee went to Urgent Care for a medical assessment where the physician noted swelling and reduced movement in the injured right knee. Physician approved returning to work the following week with moderate restrictions.

Lifting a piece of steel and something tweaked in lower back.

Tightening bolt on face plate and something pulled or popped in right shoulder.

Working on dredge chain, started feeling bad, the scale man called for an ambulance.

Trying to remove track roller on excavator. Hit it one time with a hammer. A piece broke off and went through my lip and broke my tooth.

While adjusting bearing on a conveyor, a co-worker accidentally hit employee's finger with a hammer.

Went to wash out crusher, lifted door and door stayed up, then fell back down on hand while washing out.

Walking off ladder of drege stepped on uneven steel and twisted right ankle.

Tightening bolts using breaker bar and socket and pinched finger.

Cutting belting, slipped, cut left wrist with razor knife.

Left index finger was smashed between sledge hammer and pry bar while cleaning tracks on excavator.

While switching from welding hood to hard hat, sand fell out of hard hat and blew into left eye.

Trying to free cable, pinched hand.

Standing on a 5 gallon bucket while welding and the bucket rolled. He fell off the bucket.
