Employee notified me @ 8:25 am that they had hurt their shoulder shoveling under a belt line. They said they thought that they had pulled something in their right shoulder.

Employee was hauling rock to the crusher under normal conditions. EE was traveling up a ramp from level two to level one and EE's truck stopped on the ramp. EE tried to back down the ramp and lost control of the vehicle resulting in the truck turning over on its side.

The operator was cleaning out a waste concrete pit as EE normally does and said hit a hard piece of concrete and jarred the loader and then continued to clean the pit and about 10 minutes later EE's back started hurting.

The miner was hand scaling a work face and removed a rock that hit the basket on man lift breaking it into several pieces. Some of the pieces hit their right arm and left hand. Miner said their back was also hurting because one piece of the rock hit their hard hat. Miner had a scrape on left arm and a small cut on right hand. Miner also had a red mark on their knee.

Mechanic driving a steel shaft out of bushing had a piece of metal fly off the shaft while striking it with a hammer. Mechanic had safety glasses and gloves on. Metal struck right arm and started to bleed. EE wrapped it up but it began to swell. EE went to Frankfort Regional Medical center after shift and they told EE to go to UK Sunday to get it removed.

Injured was working from aerial lift wearing harness and 2 leg SRL performing normal task, hanging steel, hanging siding, using hand and power tools, constructing new plant truck load out. Over the course of several days employee developed pain in left shoulder area. Diagnosed as strain.

Miner operating the JAW crusher called me at 11:36am saying they didn't feel good. I immediately responded and found miner in great pain. Miner was complaining of shoulder pain. I noticed miner was having symptoms of a heart attack and took them to the hospital. We arrived at the Frankfort hospital around 11:52 am.

Employee was cleaning out drill holes to be loaded. He was bent over and when he went to stand up felt something pop in his back. Was brought to the surface were he was taken to doctor. No x-rays or MRI were performed but was given muscle relaxer pills and pain pills. Was back to work on 6/7/12 but not able to do anything.

Employee had safety glasses on at the time of the accident. Employee had eyes rinsed out at the mine site eye wash station and was sent to the hospital.

EMPLOYEE HAD A PREVIOUS BACK INJURY TAHT WAS UNKNOWN TO THE COMPANY WHEN HE WAS HIRED. THE WORK THAT HE DOES FOR US HAS AGGREVATED THIS INJURY. THEIR WAS NEVER ATIME OF ACCIDENT. THIS WAS AN ON GOING PROBLEM. EMPLOYEE DOES A VARIETY OF DIFFERENT JOBS.

ALL EMPLOYEES EXITED THE MINE AT 4:30 PM ON SEPT 6 AT THE END OF THEIR SHIFT.THEY RETURNED TO THE MINE AT 4:00 AM ON SEPT 7 TO FIND OUR GROVE CRANE COMEPLETELY BURNED UP AFTER A COMPLETE INVES TIGATION BY ME CAUSE OF FIRE IS UNKNOWN.ALL EMPLOYEES EXITED THE MINE AND DID NOT RETURN UNTIL SEPT 9.THE MINE WAS COMPLETELY CLEAR AT THIS TIME.

EMPLOYEE WAS STRIKING A HAMMER WRENCH WITH A 12LB SLEDGE HAMMER. THE SLEDGE HAMMER MISSED THE WRENCH CAUSING EMPLOYEES FINGER TO BE SMASHED AGAINST WRENCH. THE SKIN ON FINGER WAS TORE BUT NO BONE DAMAGE. STITCHES WERE REQUIRED.

EMPLOYEE WERE USING A HAMMER & PUNCH TO DRIVE A PIN OUT OF A CHAIN. THE VICTIM WAS HOLDING THE PUNCH. WHEN THE HAMMER STRUCT THE PUNCH A PIECE OF THE PIN CHIPPED AWAY HITTING VICTIM IN THE EYE . HE WAS TAKEN TO HOSPITAL FOR EXAMINATION. NO ACTION WAS REQUIRED AND VICTIM RETURNED TO WORK.

I,NAME WAS USING A CUTTING TORCH TO CUT A SECTION OF CONVEYOR. WHEN THE CONVEYOR WAS CUT INTO THE SECTION PIVOTED UP HITTING EMPLOYEE IN THE SIDE OF THE BACK. HE WAS TAKEN TO HOSPITAL FOR EXAM INATION. HE HAD BRUISED AND STRAINED MUSCLES IN HIS SIDE.

VICTG M WAS ELECTROCU7ED UPON CONTACT OF UNKNOWN SOURCE. THE LOAD TRW LEVER WAS NOT THROWN OUT. HOWEVER THE TWO OTHER CONTROLS SHOULD HAVE DEENERGIZED. NO LOCKOUT WAS DONE.
