EMP WAS OPERATING BACKHOE AND STARTED FEELING NUMBNESS IN HIS ARMS AND SEEING SPOTS. GOT OUT OF THE MACHINE AND THOUGHT HE FELT BETTER AND STARTED TO GET BACK IN AND PASSED OUT.

EMPLOYEE WAS TAKING LINES OFF THE BUCKE TCYLINDER AND IT SPRAYED OIL IN HIS FACE. IT STARTLED HIM AND HE STEPPED BACKWARD AND BUMPED THE BACK RIGHT SIDE OF HIS HAD ON THE BUCKET.

HE WAS HAULING MATERIAL OUT OF BINS AFTERIT HAD BEEN WASHED. HE HAD BEEN OUT OF HT ETRUCK ONCE AND POSSIBLE GOTTEN HIS FEET WET. AS HE WAS GETTING OUT OF THE TRUCK THE SECOND TIME HE SLIPPED AND FELL OFF THE TRUCK.
