EE & 2 CO-WORKERS WERE SETTING STEEL INTO FEEDER #1. THE FIRST PIECE WAS PLACED INTO FEEDER BY CRANE OPERATER & WAS SECURELY ON FLOOR & RESTING ON SIDE OF FEEDER. SECOND PIECE WAS LOWERED INTO FEEDER, & EE & CO-WORKER WAS REMOVING SLAG FROMSECOND PIECE. EE FELT SHARP PAIN IN LEFT FOOT. FIRST PIECE OF STEEL HAD SLIPPED ONTO HIS FOOT.

EE WAS PUSHING OFF SPOIL PILE WITH DOZER WHEN HE BACKED OVER AN EMBANKMENT DROPPING APPROX. 10-15'. DOZER TILTED & REMAINED ON ITS LEFT SIDE. EE WAS WEARING SEATBELT, BUT STRUCK LEFT SHOULDER ON INSIDE OF CAB.
